Recreation participants must have a completed and approved Recreation Enrollment Packet before they are eligible to attend DSFC sponsored Recreation events. Clients that are enrolled in DSFCs Service Coordination should contact their Service Coordinator to complete the Recreation Enrollment Packet. Franklin County individuals with a developmental disability but are not enrolled in DSFCs services can download the packet from our website and call Barb Laberer at 636-5835801 ext. 1211 to make an appointment to submit the form. The approval process for non-DSFC clients may take up to 3 business days after meeting with Barb.
