MPD-SAPMT SAFETY TEAM SAFETY ALERT #74 FIRE INCIDENT - PROPERY DAMAGE

Summary: On Friday 21 May 2010 at approximately 10:40 hours, the Coast Guard portable office at Marine Control Gate had electrical short which resulted in fire. Fire destroyed the entire interior of the cabin. There were no injuries associated with this incident because the building was unoccupied at the time of the incident. Root Cause: 1. Electrical short circuit of the A/C UNIT. 2. Hot weather condition resulting in overloaded circuit. 3. Poor Housekeeping /Preventive Maintenance Immediate Corrective Actions: 1. Area was cordoned by security. 2. Duty Manager & MECC were informed for assistance. 3. Fire crews were mobilized immediately. 4. Investigations carried out to establish root causes.

Recommendations to avoid recurrence: 1. Dont overload Electrical wiring capacity. Total loads must be compatible for the wiring. 2. Always clean A/C units performing regular PM. 3. Dont use substandard electrical outlets. 4. Cooking & smoking inside portables is prohibited. 5. Eliminate the accumulation of combustible materials maintaining good housekeeping. 6. Discuss with all at TBT about requirement for fire prevention. 7. Any wiring installation should be inspected by competent electrical personnel.
