Swarna Devi aged 60 years widow of Late Shri Saroop Singh (No. 764700040 Late W/Man Saroop Singh of Border Security Forces), presently residing at Village : Churtha PO : Nerti Tehsil : Shahpur District : Kangra (H.P)-176208 do hereby state as follows :that the deponent was born on 01 April 1953 at Village and PO : Mehrna District : Kangra in the state of Himachal Pradesh. The deponent has not any documentary proof in support of her place and date of birth. Deponent do not possess any educational qualification being an illiterate person. that husband of deponent has recorded her date of birth as 01 April 1953 in his service record being maintained by Border Security Forces. I take oath and solemnly declare/affirm that the particulars furnished be me are correct and nothing has been concealed or misrepresented any facts.
