AFFIDAVIT

1, SHRI . AMRIT SUNAR, aged about____ years, son of Smti. Bhamani Sunar, resident of Jhalupara,
Shillong, East Khasi Hills District, Meghalaya, do hereby solemnly affirmed and declare as follows:

1. That I am the bona fide citizen of India by birth and resident of the above mentioned address.

2. That my name and my mother actual and correct name is AMRIT SUNAR and BHAMANI SUNAR

3.That due to my bonafide mistake my name and my mother name has been wrongly written and
recorded/reflected as " AMRIT SUMAR," and “ASHA BAHAMANI SUNAR” instead of " AMRIT SUNAR”
and “BHAMANI SUNAR” in my Birth Certificate bearing Registration No. 304/2015 date of Registration
25-06-2015 date of issue 7-8-15 issued by the Chief Executive Officer Cum registrar of Births & Deaths
Shillong Cantonment Board.

4. That now 1 intent to correct/rectify my name and my mother name from " AMRIT SUMAR," and
“ASHA BAHAMANI SUNAR” to " AMRIT SUNAR” and “BHAMANI SUNAR” in the said birth certificate
mentioned above hence this affidavit.

5. That the name i.e. AMRIT SUNAR and AMRIT SUNAR and BHAMANI SUNAR and ASHA BAHAMANI
SUNAR belong to one and the same person i.e myself and my mother.

6. That this instant affidavit is sworn for the purpose of declaring that the statement made in paragraph
2-4 are substantially correct and for submitting this affidavit before the concerned authority (ies) as a
documentary proof for necessary correction /alteration and for all other intents and purposes.

7. That the statements are true and correct to the best of my knowledge, belief and information and
nothing is concealed therein.
