(Mrs. M contacted) regarding Tyson's hitting other students. Grandma agreed that Tyson will need to stop but he is also hitting them at home. Doctor thinks its caused from his ADHD medication. (Mrs. M contacted left message) regarding Tysons behavior. He was hitting. He also cut another students shirt & hair. He was spitting at me and screaming inappropriate words. Mom called about Tyson cutting her hair/shirt. Could not get ahold of her on phone so I responded to her via agenda letting her know students were seperated. Mom called about Bella possibly having UTI so she will be going to the doctor Sat. Mom wrote an email about Elias doodling on papers and his math grade. Often he is not paying attention and rushes to get done so he can draw pictures Mom will be rewarding him at home as well as reminding him not to doodle on papers. He did not complete VPK so she feels that is contributing to the attention problem. She will be working on listening skills at home as well. Mrs. M contacted (left message) at home regarding his behavior. He ran off during PE & hit me in line and laughed about it. He came back to the room and continued to be disruptive. When Mrs. M arrived he was misbehaving then as well. She asked him to do what he was told or she would be calling home. He ignored her so she made the call and left a message. I wrote Tyson up on formal discipline. Mom wrote a note regarding Brianna's accident the day before. She has kidney problems and she wants Brianna to use the bathroom whenever possible because 9/7/2010 Tyson

it leads to UTI's. I let Mom know I would let the specials/cafeteria teachers know. 9/24/2010 Tyson Contacted Tysons mom about his day. She did not answer the phone so I left a message letting her know about his day and that he was in ISS for the day. She has not called back yet. Grandma called about Tysons conference. She said she never received a form with the time. I called Grandma back and also left a message on moms cell phone to let them know that I never received a form from them but I did have openings available that night. They never called back to schedule a conference. Grandma called to see if Tyson had a good day. He had forgotten his agenda in school I informed her he did and she was happy. Her mom called to find out about purchasing PTA carnival tickets. I told her to send the form in she had received. Grandma called twice about Tysons missing sweatshirt. I asked if he had his name in it but he did not. I told her (as did Mrs. M twice) that we would look for it. Aunt contacted me to find out how Jackies behavior was and what I thought my be causing the behaviors. I told her she just gets mad when she doesn't get her way. Mom contacted to let me know Kyle was going to the Doctors. Follow up-He has bronchitis but he is not contagious. Mom contacted me about Caleb's progress. I let her know he continues to need lots of practice & additional help. I gave her things to work on with him at home. Mom contacted me about class supplies she wanted to know what else we needed for our class. I told her we currently need only glue sticks. Grandma contacted me twice to let me know he was having trouble with another student bothering him on bus. I let Teresa know about the situation. Mom wrote a note to confirm Dad was an approved volunteer and would be coming to the Gingerbread Gathering. Spoke to Aunt about Jackies behavior. She said Mom is still having issues so Jackie is

not seeing her right now so she believes that is what is contributing to Jackie's behavior. Mom called because she said Taina told her a boy from Mrs. Stoessel class touched her in appropriatly at recess. I told her I would contact Mrs. Jones right away and she would get in touch with the mom right away. Mom wanted to know how IEP meeting went since she missed it. I filled her in on it. She also wanted to let me know she has first counseling session today. Mom also said she is not sure why Emmah is misbehaving in class again. Mom wrote a note about placing Kyle in the front row of class pictures so he could be seen. Aunt wrote a note that Jackie will be staying with Mrs. Barnum next week and she has guardianship next week while the family is on vacation. Mom stopped by to see about bringing a cake on the 100th Day of school so the class could celebrate. I told her it would be fine but we cannot have 100 candles! Mom contacted me about her receiving ESOL services. She wanted to know if she was considered ESOL and I said no she would have received information if she had qualified. Mom contacted me about putting Hailey's report card in a sealed envelope because she wasn't going straight home. I let her know I would. His mom wrote to see if we had extra homework pages because he misplaced his. Mom contacted me about Caleb's grades and why he was failing some subjects. I called her back to discuss possible retention. (see rest of phone conference notes in file) Let Grandma know that he was having difficulty getting work done in school again. She said Mom (who he doesn't live with) is having baby and she hasn't been spending much time with him she thinks that is effecting the way he is acting. Mom let me know Emmah would be wearing eye patch every day and also let me know she would be having operation on tumor in her head soon. She would let me know when she finds out the date. Let Mom know I was seeing lots of progress on Caleb's blending/letters! Grandma contacted about having Nathan tested for ADD and what my thoughts were about it. I talked to Teresa and responded back to Grandma. Mom called to let me know Emmah would be leaving early to get her pictures taken at 9:00 Mom called to let me know Taina was sick and maybe some one in the class could be really sick as well. Wrote Mom a note to inform her that Emmah received a discipline for kicking 2 students
