LOCATION OF POSITION: 1904 SE Division Street Portland, OR 97202 HOURS: SALARY: BENEFITS: Full Time Starting at $15.20 / hour Medical, dental, vision, long term disability

I. MAJOR DUTIES AND RESPONSIBILITIES: Position Description Provide one-on-one support to high-risk children/youth and their families in the home and community. Includes the provision of supervision for safety, assistance in accessing and participating in community activities, and mentoring and coaching positive social skills. Work as a team member with other professionals. Position requires 24/7 on-call availability. Position Responsibilities 1. Universal job skills: All CCS Family Preservation employees will demonstrate the ability to: a. Communicate role, responsibility and agency philosophy in order to assure mutual respect, confidence and trust with the child and family and other stakeholders. b. Provide non-judgmental, unconditional support to the child and family. c. Assess for immediate safety and stabilization needs. d. Customize helping approaches to fit the familys uniqueness, personality, culture and interest. e. Report relevant information to the right people at the right time. f. Maintain a focus on strengths, needs and creative solutions. g. Utilize the familys expertise in problem solving and solution seeking. h. Work interdependently with others toward common goals.

i. Respond to familys and childrens needs in a timely fashion. j. Complete required documentation within established timeframes. k. Instill hope for the future by communicating and behaving with confidence and reassurance. 2. Welcome, Engagement and Assessment: The skilled Family Preservation FSS will demonstrate the ability to: a. Communicate role, responsibility and agency philosophy in order to assure mutual respect, confidence and trust with the child, family and other stakeholders. b. Assess for immediate safety and stabilization needs. c. Meet and engage the family communicating compassion, support, respect and enthusiasm for them and your role as a helper. d. Support family during stabilization by providing rest, relief and safety. e. De-escalate risky situations safely, effectively and confidently. f. Communicate and coordinate with other involved staff. g. Analyze and assess environmental and personal situations in work with families as it relates to safety, basic needs, unmet needs and other areas. Individualized Service and Support Planning: The skilled Family Preservation FSS will demonstrate the ability to: a. Provide non-judgmental, unconditional support to the child and family. b. Customize helping approaches to fit the familys uniqueness, personality, culture and interest. c. Provide input and feedback in the development of the plan. d. Consult with supervisor and Therapist when needed. Implementing Services: The skilled Family Preservation FSS will demonstrate the ability to: a. Report relevant information to the right people at the right time. b. Maintain a focus on strengths, needs and creative solutions. c. Work interdependently with others toward common goals. d. Respond to child and family needs in a timely fashion. e. Complete required documentation within established timeframes. f. Seek and solicit direction from the Therapist involved in the development of the plan. g. Reach agreement about the planned activities for the FSS across the following areas: i. Model and teach appropriate social behavior and skills. ii. Connect and introduce youth and family to community resources. iii. Identify and access positive relationships for the child and family. iv. Provide direct supervision and redirection of children. v. Build youth capacity. vi. Other areas as called for in the plan. h. Communicate and coordinate with the parent and others about schedule, location and assigned activity. i. Go to the assigned location for service delivery at the assigned time. j. Assess the immediate situation and compare it to the planned activities.

k. Choose a method for implementing planned activities including either: i. Adapting the chosen activity to fit the family situation; or ii. Implementing the activity as planned. l. Communicate and summarize implementation or modification to the Therapist. m. Seek assistance and report incidents to the right people at the right time. Transitioning Services to Natural and Other Community Supports: The skilled Family Preservation FSS will demonstrate the ability to: a. Utilize the familys expertise in problem solving and solution seeking. b. Instill hope for the future by communicating and behaving with confidence and reassurance. c. Choose and implement transition activities based on child/family strengths, primary lead feedback and anticipated future need. d. Reinforce child and family gains in interactions with them. II. QUALIFICATIONS: A. Minimum Qualifications 1. Age 21 or older per DSHS requirements. 2. Two years relevant experience with youth and families 3. Ability to make visual assessments of family home conditions and crisis situations. 4. Qualified Mental Health Associate (OAR--# 309-032-1110 (71). (See below) 5. Must have reliable transportation, valid drivers license, automobile insurance and clean driving record. 6. Must provide acceptable driving abstract per driving policy. 7. Pass background checks through DHS, WA State Patrol and OR State Patrol. 8. Availability to work flexible hours (evenings, nights, weekends). 9. Demonstrated ability to provide skill building services to youth. 10. Demonstrate effective verbal and written communication. 11. Ability to work effectively within a team. Oregon requires applicants to be a Qualified Mental Health Associate (QMHA). Requirements for QMHA are defined under the current OAR as either A or B: a. Experience: A combination of three years experience, work, education or training revolving around child mental health, child development and/or social skill development. b. Education: a Bachelors degree from an accredited college or university with a major in the behavioral sciences field. And: Communicate effectively, understand mental health assessment, treatment and service terminology and to apply the concepts, provide psychology skills development, and implement interventions as assigned on an individual plan of care.
