Dear Sir/ Madam: On November 21, 2012 I sent to Manulife Financial Travel Insurance c/o Ms. Melanie Venables (Meloche) Claims Examiner of Active Care Management some documents related to the Manulife Travel Insurance Case # 47393 of my late wife Ofelia Fonacier Montefrio. The documents forwarded to your office included the following; 1. Photocopy of the cover letter addressed to Manulife Financial Travel Insurance dated November 21, 2012. 2. Photocopy of my letter addressed to Dr. Eduardo M. Jamora, M.D., of the Comprehensive Pulmonary Clinic, Buendia, Makati City, Philippines 3. Photocopy of my letter addressed to his Excellency, The Ambassador of Canada in the Philippines. The content of the said letter in self explanatory. 4. Medical records of HORACIO and OFELIA MONTEFRIO released by Dr. Ariel Miranda Attending Cardiologist of the Santos Medical Center, San Juan City, Metro Manila, Philippines. Said medical records where submitted to Comprehensive Pulmonary Clinic and subsequently forwarded by Comprehensive Pulmonary Clinic to the Canadian Embassy of the Philippines. The results of the medical tests (Treadmill Exercise Test and Echocardiogram) of OFELIA. Unfortunately the medical records released by the Cardinal Santos Medical Center to Comprehensive Pulmonary Clinic were not included in the documents sent to you last November 21, 2012 via FedEx. I am therefore forwarding to you copies of the medical test results of the Treadmill Exercise and Echocardiogram of OFELIA. Again, I hope and pray that the Canadian Embassy in the Philippines will favorable consider my request for the release of all the medical records of OFELIA Submitted by Comprehensive Pulmonary Clinic. Incidentally I am wondering if it is possible for Manulife Financial travel Insurance to officially request the Canadian Embassy in the Philippines for the release of the

medical records of OFELIA. I am confident that it will be favorable acted upon for the reason that MANULIFE is a Canadian Company. Thank you for your favorable consideration and action. More power to Active Care Management of Manulife Travel Insurance.
