COMMENTS
 ELCB is not installed in DB. In case any consumer gets earthed, there is no
protection.
 There is only a two phase supply on DB instead of three phases.
 There is very poor illumination in the hall as only three light fittings are
working. The total number of installed fittings are 16.
o As no voltage drop was noticed, the reason for not working of these
fittings is either
 Defective wiring and/or
 Fittings are defective (Ballast, starter tube)
o This needs to be checked urgently.

 There are no legends of DB wiring. This is making trouble shooting difficult.

 Power cables each 1Cx 8mm are tapped from incoming of DB Isolator. This is
to feed power to GF DB, installed for security changing room and toilets. As a
result, DB looks jammed and unsafe for maintenance and troubleshooting.

COMMENTS
 There is no incoming power to DB for the last several months. Therefore,

the bulbs, fans etc are not functioning.
 Incoming power is tapped from the main hall DB instead of the bus bar panel.
 Wiring has a problem so it needs to be fixed immediately.

 There is no legend of DB wiring.
 Size of Isolator is very high and so are the sizes of MCBs, as connected load
is very low.
 Two MCB are missing from DB.
 There is no protection equipment installed to protect the system from earth
leakage current

 AC Split Units - 6 (1.5HP,PANASONIC)

COMMENTS
 DB is powered with two phase supply only. only.

 Similar to ground floor DB, two phase power is tapped from incoming of the
Isolator to feed power to kitchen DB.
 There is no legend for electrical wires going out from the panel
 DB wiring is clumsy, making it difficult to measure voltages and load current.
 Since most of the time there is no power, the fire alarm system remains
mostly non-operational. This is because its UPS can only sustain for half an
hour.
 No emergency light was seen in the big hall.
 The illumination level is low as most of the fittings are non-operational.
 13Amps SSOs are connected in ring with 2.5 mm2. These were found to be

 Similarly, 15Amps SSOs connected in radial with 4mm2 wire is OK. OK.

COMMENTS
 The ring wired circuit of single 13Amps socket outlet supplies power to
o Refrigerator
o Microwave oven
o Water dispenser
o Electric Kettle.
 The power cable between 13A SSO to consumer is sagging.
 All bulbs were found to be fused.
 No earth leakage protection equipment is mounted in the incoming DB.
 There is only two phase incoming supply at DB Isolator, tapped from first floor
main hall DB isolator, instead of a three phase supply from bus bar panel.
 No legend of DB wiring.
 Size of DB and Isolator is high.

 AC Split unit - 2 (3phase,4.8KW power)

 Louver fitting - 16 (4x16W)
COMMENT
 Three phase power is tapped from Isolator incoming for another DB.
 Also three phase power is tapped for supplying power to Air conditioning units
 No earth leakage current protection system is installed.
 Only one split AC is working while second one’s compressor is not functional.
 Light illumination level is low as most of fittings are non-functional. It needs
wiring and fitting to be checked and fixed.
 There is no emergency light installed.

[Changing room]

COMMENTS
 DB incoming supply is tapped from second floor main hall DB Isolator’s

 No ELCB is installed in DB to protect the system from earth leakage current.
 Isolators and MCB are oversized.
 In general illumination level is low.

EXTERNAL ELECTRICALS

COMMENTS
 The perimeter lights are all in one circuit (radial), terminated to a 32Amps
gear switch.MEM Gear Switch is mounted on the fence behind the main
building.
 The fittings are looped with a 2.5 mm2 PVC/SWA/PVC cable and they are
terminated with 3c x 1.5mm2 industrial flexible cords.
 The poles are not earthed.

 Termination at the JB is not neatly done.

 Some of the 6” x 6” boxes used to conceal the connections between loops
and fitting rid connectors are not earthed, not covered and are not made in

 Some of the fittings are dirty and have debris inside.
 Some of the fittings mounted on the security posts are unearthed and cables
are flying around the post, not concealed.
 33KV NEPA lines are passing through the base yard. The overhead line is
recommended to be replaced with underground XLPE cable.
 One perimeter fitting is broken in pieces and is hanging behind the diesel
tank.
 Junction boxes appear to be IP55 but water was seen inside.
 There is no terminal strip inside the junction boxes. Wires are directly joined
using pliers.
 No PVC gland is used; therefore, the possibility of water, dust and insects
entering the region is high.
 Galvanized 20mm conduit is recommended in place of existing plastic PVC
conduit pipe.
 External wall mounted bulkhead fittings (2) are hanging. It is IP41, while it
should be IP55.

GENERATOR SET

COMMENTS
 There is no electrical lighting point in generator room.

 There is no emergency light installed.
 The following indicators in the Genset are nonfunctional
 Engine RPM meter
 Engine HR meter
 Engine Oil Pressure
 Generator Frequency Meter
 Generator Voltage Meter
 Engine speed and thus frequency goes on changing as it is controlled

manually with two naked wires joint together, which often get loosened.
 During the audit period, frequency got down twice.
 Genset body is not earthed at all. No earth pit and earth chamber could be
located.
 There is no separate earth cable from Genset to 63 Amps gear switch
installed in Genset room.
 4C Cable AL armor is used as earth cable from 63A gear switch to 250A
Change over Switch.
 25mm2 cu armored cable is sagging/hanging in changeover switch room.

CHANGE OVER SWITCH
COMMENTS
 There are two 250 Amps 3pole breakers installed inside the 100Amps BILL
changeover panel. One is connected to the Genset while the second breaker
is connected to PHCN 16 mm2 1C AL cables. There is NO interlock
(Mechanical or Electrical).In
Electrical).In case both the breakers are ON at the same
time, it might cause serious problems.
 Bottom plate of Change over switch is removed enabling easy access to

animals and insects. This can lead to short circuit.
 No cable gland is used for generator cable connection.
 No cable trucking is used for PHCN cable and generator set cable.
 Power cable for external lighting is directly connected to Change over switch.
 External cables from Change over switch are hanging /sagging in change
over panel room. No trucking is used.
 Changeover switch of 250Amps is highly oversized for 65A maximum
capacity Genset.

COMMENTS
 Top cover is open and cables from gear switch to bus bar panel are not well
connected.
 Cable wire color code is not correct. Black wire is used for Y phase and the
same color is used for Neutral also.
BUS BAR PANEL
COMMENTS
 In coming cable insulation is over heated.

 However, top entry of cable is properly connected.

EARTHING SYSTEM
COMMENTS
 Earthing pit and earthing chamber could not be located therefore earth
resistance could not be measured.
 None of the electrical panels like DBs, Gear Switches, Bus bar panel, Genset
body are properly earthed.
LIGHTENING ARRESTOR
COMMENTS
 Lightening arrestor is not connected to the earth chamber. Rather, an earth

copper strip is directly going to earth. Earth resistance could not be measured
as there is no earth chamber.

FIRE ALARM PANEL
COMMENTS
 It has been observed that there is frequent power failure as PHCN power is
disconnected for the last two months and the Genset remains under
breakdown very frequently. Last month there was no power for more than two
weeks. Under such a situation the fire alarm system will be nonfunctional as
its UPS could last only half an hour. In case of a fire, the system will fail and
there would be serious consequences.
