3. CSC Certificate of Eligibility (authenticated);
4. *Transcript of Records;
5. *Certificates of Seminars or Trainings attended;
6. *Service Records or certification from previous employers stating the position assumed and corresponding duties
and responsibilities;
7. Performance ratings in the last two (2) semesters (for government employees only).
