Director of Social Services
• Manage a staff of 5 Social Services assistants who were responsible for coordinating the health care needs of 230
residents.
• Complete comprehensive social history assessments, psycho-social assessments, and behavioral screenings on all new
admissions and hospital returns.
• Facilitate Care Conference, Discharge Planning, and Family Conferences to maintain the continuity of care for all
residents.
• Builds relationships between residents and staff by teaching staff how to understand and support residents.
• Participates in professional development, such as attendance at facility in-services and outside conferences and
