Some lumber (bundled square logs) that has been loaded onto a truck is starting to

fall off the rear. Workers are trying to use the bucket of a tractor shovel to push the
logs back in place using a prop. One worker is riding in the bucket to help do this.

The tractor shovel suddenly lurched forward, causing the worker to be crushed
between the lumber and the bucket.

Tips for preventing similar accidents


1. Do not ride in machinery buckets or anywhere other than the operator's seat.
2. Refuse to operate machinery while other workers are riding anywhere other than
in the operator's seat.
3. Use the proper equipment for the job, such a crane, and do not use machinery for
purposes for which it is not intended.
4. Ensure that all workers are thoroughly trained in safety procedures, including
activities that are prohibited at work sites.

Dirt has been produced at a nearby construction site and is going to be reused at a
rock quarry. A wheel loader is therefore being used to smooth out the piled up dirt.

The weight of the vehicle caused the edge of the pile to collapse. The vehicle tires
came off of the shoulder and the vehicle tumbled down from the top of the pile.

Tips for preventing similar accidents


1. Machinery operators should know shoulders are unstable. Before starting work,
check the shoulders and perform a safety inspection of the footing.
2. If dirt is in danger of collapsing, halt work immediately and take appropriate
safety measures. For example, workers should place a flag, pole or other sign
showing the location of the shoulder.
3. The driver should check the positions of the forward and reverse levers, and work
perpendicular to edges when performing dirt pushing work.
4. Select the appropriate vehicle (e.g. crawler-type) for the ground and other

conditions of the earthwork being performed.


Workers are assembling a plastic greenhouse. A tractor shovel is being used to
move a bundle of pipes for the frame while a worker steadies the bundle.

The worker who was steadying the bundle tripped and fell over a ridge in the
ground, and was run over by the front wheels of the tractor shovel.

Tips for preventing similar accidents


1. When moving suspended loads, use a mobile crane or a vehicle-type construction
machine with a crane attachment. (Some types of moveable cranes can be moved
while carrying loads.)
2. When loading or unloading long materials, use a guide rope to guide the materials
from beyond the operating radius.
