South, I have residing at Karachi since birth continuously remain accompanying
with my family of her life.

That my late father name is Noor Mohammad S/o Haji Mohammad Umar,
NIC No.00450-3406512-7, he was expired on 14 November 1996 Certificate is
attached herewith.

That my mother name Mst. Zubaida W/o Noor Mohammmad D/o
Mohammad Rafique NIC No.450-46-065128 she was expired on 08 October 1985
Certificate attached herewith.

That my brothers namely Shabir Ahmed S/o Noor Mohammad NIC
No.42301-3701217-5 my real brother relation with real sister and brother.

That my brother Amir Khan S/o Noor Mohammad NIC No.41301-6273258-9
my real brother relation with real sister and brother.
Sir my case is very sympathetically reason known to us NIC objection. Kindly
confirm verify maintain in your record of NADRA.
I, therefore, request you to kindly necessary orders in my favour for issuance
of National Identity Card as per NADRA Rules & Regulation Policy.
