As parents/guardians of ___________________________, do hereby give our consent for
him/her to travel and to attend the activity, The Apprenticeship Program (TAP) on June 11,
2016 at Multi-function Room, Albertus Magnus Building, University of Santo Tomas,
Espaa, Sampaloc, Manila as part of the programme of the officers of the Philippine Council of
Hospitality Management Students (PCOMHS). For the duration of his/her stay in the said place,
our child will be responsible enough for his/her own accommodation and travel expenses.
We (I) do hereby release, forever discharge and agree to hold harmless the officers and the
organization with the rest of its members and staff from any and all liability, claims or demands
for personal injury, sickness, or death, as well as property damage and expenses, of any nature
whatsoever which may be incurred by our son/daughter that occur during the above trip or
activity.
This Letter of Consent is being issued in compliance with the organizational policy and
regulation for students traveling not accompanied by or joining either parent/guardian.
