MEMORANDUM OF AGREEMENT

KNOW ALL MEN BY THESE PRESENTS.

This agreement made and entered into this _____ day of ________ 20____in
____________Philippines , by and between:

_____________________________, herein known as the Teacher Education Institution (TEl),
also referred to as the First Party;

AND

Department of Education, Bohol Division, a national agency organized under the


Republic of the Philippines, with its business address at Lino Chatto Drive, New Capitol Site,
Tagbilaran City, Bohol, represented by the SCHOOLS DIVISION SUPERINTENDENT, EVANGEL
M. LUMINARIAS PhD, CESO V, herein referred as the Second Party;

WITNESSETH

WHEREAS, both parties are desirous to improve the quality of education in elementary and
secondary schools in the Province of Bohol with the following common goals and objectives of
the practice teaching program:

1. To provide opportunities for student teachers in applying real and actual situations
of the principles learned regarding the teaching-learning process, to evaluate
themselves through various instructional tasks, and discover their strengths and
limitations in their journey throughout the student-teacher program;

2. To gain a comprehensive and realistic picture of the Philippine instructional system


which a student -teacher envisions to gain employment, and career growth;

3. To acquire classroom management skills and an understanding of the real teaching


challenges in order to adapt to shifting classroom observation needs.
WHEREAS, the privileges, duties, and responsibilities of the TEI are to:

1. Determine the student teacher's suitability and readiness for the of off-campus
program;

2. Allocate and collect the Practice Teaching Fee from each Student Teacher. The said
fee is to be included as a regular school fee billed on the semester of the student-teaching
practice;

3. Conduct regular supervisory visits during the stay of the student teacher in their
School;

4. Gather feedback from the cooperating schools and teachers regarding student-
teacher performances, as basis for improving the pre-service education of the
prospective teachers ;

5. Coordinate and arrange the collection of Honorarium to be given at the end of the
semester for the cooperating teachers and principal of the school where the student-teachers
have served;
6. Issue a Certificate of Recognition to the Principals and Cooperating Teachers who
have supervised the student-teachers under their care;

WHEREAS, the effectivity of this AGREEMENT shall take effect immediately upon signing by all
parties concerned and shall remain in force until the completion of the assignment and upon
the satisfactory fulfillment of all terms and conditions embodied herein unless sooner
terminated by mutual consent.
