Dear Sir,

We hereby request you to register in our favour the title/s which we intend to utilize in connection with our
forthcoming picture/s.

e) Documentary / Short Films

2. We submit herewith remittance of Rs. __________/-* in cash /cheque being the fee for registration of
the above title/s in our favour.
3. It is clearly understood by us that:-
(a) The acceptance of this application and fees does not guarantee the allotment of the
title/s as applied for. We understand that the title/s will be registered in our favour
subject to final clearance from the Title Registration Committee and also if no objection
is received from any source within the stipulated period of fifteen (15) days from the
date of the circular.
(b) We will not announce, publish or exploit the above title/s in any manner whatsoever until the grant
of the title/s in our favour is confirmed in writing.
(c) The above title/s, if allotted in our favour, will be valid for one year only from the date of its
registration.
(d) We are full aware that strictly we have to use uniform letterings, i.e., letterings of equal
size letters in all the words of our title/s, if allotted in our favour.
(e) We are fully aware that the title/s, if allotted will be for our exclusive use only and we shall have no
rights whatsoever to either transfer the same or allow anyone else to use the said title/s.
(f) In case if we fail to renew the title/s with prescribed renewal fees before the expire dated and/or if
you do not receive any communication from us in this behalf on or before the date of expiry, then
you may presume that we are no more interested in the renewal of the above title/s and you will
absolutely free to allot the above title/s in favour of any other applicant as per your discretion.
(g) We are aware that out applications for new title/s will not be accepted at the eleventh
hour and we abide to follow the procedures of applying for new title/s before the
commencement of our film projects whatsoever.
(h) We shall strictly adhere to the Rules, Regulations and Guidelines of the Title Registration
Committee at present existing or as may be amended from time to time and/or any decisions given
under the said rules, regulations and guidelines will be binding on me/us at all times.
(i) We understood that the above title/s is/are being registered with you in order
to facilitate us by mutual arrangements between film fraternity. We also
understood that you have no legal liability or responsibility upon the said
registration of titles. We hereby undertake that we shall indemnify you from
any legal consequences.
(j) We understand that if the above titles applied are already registered in favour
of anybody else or registered under Trademark or any human error occurs
while allotting titles, you have no liability or responsibility of whatsoever
nature on any dispute/claim arises out of the above title allotment .
- Sir, we fully understood the above and will abide by all the
said clauses.
