Responsiblities of the Handle, Assist and Cord Care

(St. Anthony Mother and Child Hospital)

Handle:
Do labor watch and take the vital signs every hour (or every 30 minutes).
Assist the mother when she is about to be mounted on the DR table at the
appropriate time.
Assist patient in Lithotomy position.
Perform perineal flushing on the mother.
Scrub hands thoroughly and wear sterile gloves.
Get the sterile cloth from the sterile set. This is used to support the perineum.
When the mother is about to bear down and the babys head is about to go
out, support the perineum by pushing ones palms with the cloth firmly on
the perineum until the baby is fully delivered.
Watch out for the delivery of the placenta, noting its mechanism of delivery.
Recieve it using the sterile tray from the sterile set used.
Dispose the placenta in a plastic bag and put Zonrox on it so as to minimize
the foul odor. Place the plastic under the sink.
If episioraphy is finished (if there is), do after care and perform perineal
flushing.
Put on the mothers diapers.
Wipe the nipples of the mother with an OS soaked in water in preparation for
latching on.
After latching on, assist in changing the mothers clothes. Prepare her before
her transfer to the ward.

Assist:
Assist in doing labor watch and take the vital signs every hour (or every 30
minutes).
Once the patient is mounted on the DR table, scrub hands thoroughly and
wear sterile gloves.
Have the circulating nurse unwrap the sterile set, and after it is unwrapped,
arrange the instruments on the sterile tray.
Hand the sterile cloth to the handle.
During the delivery, hand the needed instruments to the doctor, nurse, or the
cord care.
 Note the time when the baby is out, and the time when the placenta is
already out.
If episioraphy is to be performed, assist the doctor by handing the needed
instruments.
After everything is done, do after care by washing all the used instruments
carefully, and then placing them in the work room/drying room afterwards.
Assist in changing the mothers clothes and putting on her diapers.

Cord Care:
Prepare all the clothes of both the mother and the baby.
Scrub hands thoroughly and wear sterile gloves (double gloving).
When the baby is already out, clamp the umbilical cord (mothers side) with
kelly curve forcep and the umbilical cord (babys side) with kelly straight
forcep.
Cut the cord between the forceps.
Using the umbilical cord clamp, clamp the portion of the cord 2 cm directly
below the umbilicus of the baby.
Turn on the gooseneck lamp and place the baby in the cradle. Dry the baby
immediately.
Obtain the babys weight, length, head circumference, chest circumference,
and mid-arm circumference.
Obtain the babys vital signs. Note the APGAR score in 1 minute, then after 5
minutes.
Perform cord dressing.
Administer 1 drop of gentamicin (Credes prophylaxis) on each of the babys
eye.
Administer 0.1 cc of Vitamin K on the babys left anterolateral thigh through
IM route.
Administer 0.5 cc of Hepatitis B vaccine on the babys right anterolateral
thigh through IM route.
Attach identification band bearing the babys name, time and date of
delivery, and mothers name on the babys right leg, blue for boys and pink
for girls.
Sign the baby card after administering the Hep. B vaccine.
Dress the baby after everything has been performed. Prepare the baby for
latching on.
After latching on, carry the newborn while accompanying the mother to the
ward. Endorse the baby properly.
Do after care.
