Ns. Dewi : I want to report a patient in Mawar Room, Doctor.

Doctor : how about the patient's data, nurse?

Ns. Dewi : Ms. Dini, 20 years old, Come to Hospital with diagnoses painful Diabetic
Neuropathy in his feet, obesity, CAD and PVD. I have done give some medicine
like as you told me yesterday like 6-8 acetaminophen 30mg codein/days,
amitryptyline 25mg hour sleep and gabapentin 100mg, 3 times/days for .

Doctor : okay, how about the patient's vital signs?

Ns. Dewi : Vital sign of Ms. Dini is Blood Pressure is 170/100 both arms, Pulse is 85
regular, Temperature is 37, chest clear; no pulses in feet, thickened nails, feet
cool; loss of sensation in feet up to midcalf to touch, pinprick. Extreme sensitivity
in feet , constipasi and nausea.

Doctor : How about her pain, nurse?

Ns. Dewi : She feel pain in his toes and the bottom of his feet with pain scale 8/10. She has
electric shock pain about every 2 hours in feet and up his legs with pain scale
10/10. How About this Dr. Arini? Should we have to take some more medication
for Ms. Dini for her pain?

Doctor : okay Ns. Dewi, still give the amitryptyline and gabapentin in same dosis for the
pain and Duloxetine 60 mg/day too. Still monitor the patient's blood glucose
levels and also monitor the patient's diet program. Ask to the nutrionist about diet
program and food for the constipasi for Ms. Dini.
