Artifact 5: Hospice Report

At 1:00 on September 26, 2014, I visited Mr. A. in Newberry with my supervisor. Since this was
my first visit with the patient, my supervisor introduced us to each other. Furthermore, we sat
and talked with Mr. A. for about an hour, and together we brainstormed future activities I could
do with him. My supervisor also introduced me to the some of the staff at the Newberry facility.
