Appraisal Schedule

Home visitors will have weekly or biweekly one-on-one

supervisions depending on the need of the home visitor and/or
coordinator.

Formal observations will be performed for home visitors by the

coordinator three (3) times a year. Observations will be made in
four areas: home visits, staff meetings, group meetings,
and general. .

Annual Evaluation will be completed at the end of the year after

all other observations have been conducted and it will be from
those observations where your annual evaluation data comes
from

Typically you will be observed during the following months:

Observation 1- November
Observation 2- February
Observation 3- April
End of Year Appraisal- May/June
