AFFIDAVIT

I, ALBEN T. MATAS, of legal age, Filipino, single and a resident of Green

Field Home Owner Association, San Roque, Zamboanga City, under oath, do
hereby depose and state that:

I was a former Property Custodian of JESSK Security Agency Inc., a security

agency organized under the Laws of the Republic of the Philippines with office
addressed at Old highway Guiwan, Zamboanga City and now presently employed
at Good Shephered Janitorial and Manpower Services Corporation as an
Administrative officer;

That the agency was issued cease to operate order issued by Supervisor Office
for Security and Investigation Agencies, (SOSIA) on October 2, 2015 on the
grounds that the agency failed to renew its License to operate;

That the said security agency has already been closed since the order was
served to the agency;

That as a property custodian, I was the in charge of all the equipments of the
agency and one of the JESSK Security Agencys Equipments is ICOM ICV85
with Serial Number 1202b03006 that was caught by the National
Telecommunication Commission (NTC) last July 11, 2016 Medi-Spa, Unicahan
Building because of the violation which is without license;

The said equipment is now for safekeeping under my custody at Good

Shepherd Janitorial and Manpower Services located at 109 Old Highway,
Guiwan, Zamboanga City since Good Shepherd Janitorial and Manpower and
JESSK Security Agency Inc. are sister company;

The said equipment will get a license when time comes that the agency uses
it again; and

I am executing this affidavit to attest of the foregoing facts and to whatever

legal purpose this may serve under the law.

IN WITNESS WHEREOF, I have hereunto set my hand this 14 th day of
