AFFIDAVIT

I, ELPEDIO A. IBACARRA, JR., of legal age, married, Filipino, and with office
address located at Banadero Highway, Ozamis City, under oath aver:

1. I am the appointed as temporary Branch Manager of Davao Security and

Investigation Agency, Inc. – Ozamis City Branch (DASIA – Ozamis City Branch
for brevity), located in the aforesaid address;

2. Sometime on October and November of 2016, a certain Mateo J. Elumbaring

went to my office at the aforesaid address, introduced himself as an employee of
DASIA – Cagayan de Oro City Branch (DASIA – CDO Branch for brevity)
employed as Security Guard and requested that he be assigned at my branch;

3. I immediately heeded to his request but as a protocol and courtesy to the branch
manager of DASIA – CDO Branch, I requested him to secure a clearance from
DASIA – CDO Branch where he is currently and actually assigned;

4. However, until this date, Mateo J. Elumbaring did not report back at my office
and submit the required clearance in order for him to process his request to
transfer;

5. I am executing this Affidavit to attest and declare the truth of the foregoing facts

and for whatever legal purposes that this Affidavit may serve.

IN WITNESS WHEREOF, I have hereunto set my hand this ___ day of July
