To assist the recommender in representing you as accurately as possible in the letter, please read and respond
to the statements and questions below. The care and thought you give in addressing these prompts may
reiterate what the recommender already knows about you or may help them discover something previously
unobserved. In any event, take nothing for granted.

1. Explain the qualities that you feel you possess which might easily go unnoticed by others.
There are certain qualities about me that go unnoticed by my classmates. These qualities include my
compassion towards others, my integrity on my beliefs and values, and lastly my competence and
diligence through work to achieve my goals.
2. Complete the following statement as clearly and directly as you can: I am especially proud of the
following achievement.
I am especially proud of the following achievement. This achievement is the ability to stay on top of my
work for the past 3 years to keep a high GPA. I am proud of this because it reflects all the work that I
have put in throughout my high school career.
3. Mention any factors that have come up during your high school career which you feel have put you
at a disadvantage.
A factor that came up during my high school career that put me at a disadvantage was the countless
school nights and weekends that I had to dedicate to moving to a new home. This was a disadvantage
because I had to find time to finish my schoolwork, to make sure I was keeping up, when there wasnt
too much time available for me.
4. Do you feel your present grades are a fair representation of your academic ability? If not, discuss.
Yes, I believe that my present grades are a fair representation of my academic ability because I put in
the effort that is required to attain these grades. I strive for the A in every class to show myself that I
am giving the necessary effort to achieve these grades in each class.
5. What goals have you set for yourself and what are you presently doing to achieve them?
The goal that I have set for myself is to graduate High School with a 3.8 GPA or higher. To achieve this
goal, I put school first. I strive to meet all of my school responsibilities to the best of my capability in
order to make room for mistakes that can be fixed. I turn in all my homework on time, study hard for
challenging tests, and take higher level classes that will help my GPA with an excellent grade in the
class.
6. What activities have you pursued and to what extend? Mention any offices you have held and
awards won. Tell what you gained from this experience.
Some activities that I have pursued are Running, Gardening, and Snowboarding. I have pursued these
on an entry level. However, I have learned from each of these activities. From running I learned that
being diligent is very important, and that giving up will not be beneficial. From gardening, I learned to
appreciate the beauty of nature, and how important it really is. Lastly, from snowboarding I learned
that activities can be extremely difficult, but being consistent will help me do better the following time.
7. In what community activities have you participated?
I have participated in many community activities through my church. These activities include tutoring
at Good News Gang, being a group leader at Vacation Bible School, helping put together a fundraiser
for less fortunate pregnant women in Detroit, etc.
8. Explain your personal contribution to the high school community.
My personal contribution to the high school community is having school spirit on spirit weeks,
fundraising for necessary causes, contributing to Charity Week, supporting teams at their sports
events, etc.
9. What are your hobbies?
My hobbies include reading, running, baking, cooking, babysitting, painting, organizing, calligraphy, and
photography.
10. Describe any independent study or special programs/projects in which you have engaged (science,
drama, music serious extra work in a subject.)
Some special programs that I have engaged in have been tutoring children with the Good News Gang,
shadowing a Physician's Assistant at a hospital, and researching Sex Trafficking for a large project with
a group.
11. Describe any work experience you have had paid or unpaid. Describe your responsibilities, dates of
employment, average hours per week, etc.
Some work experience that I have had has been babysitting, house cleaning, and landscaping. My
responsibilities for babysitting are keeping the children safe and out of trouble, while also keeping
them entertained and fed. My dates of employment for this is usually weekends and Friday nights
working at least 10 hours each week. My responsibilities for house cleaning is to go through each room
of the house and deep clean and organize them. My dates of employment for this job is usually in the
summer when school is finished with average hours being at least 5 per week. My responsibilities for
landscaping is to tidy up the yard, plant new flowers and bushes, water the plants, and place mulch
throughout the yard. My dates for employment for landscaping is during the early months of summer
with an average of 15 hours a week.
12. Describe your summer experiences and what you gained from them:
a. After freshman year: After Freshman year, I got my first job babysitting. From this I learned
that it is important for me to do work so that I can build up my diligence and to learn how to
manage my time wisely. That job taught me how to keep myself on top of my duties the
following school year, and to manage time effectively.
b. After sophomore year: My summer after Sophomore year was eventful in the instance that I
traveled throughout the state and country, while also keeping my babysitting job. I learned that
I can enjoy my time with family and friends, but I am also able to keep a job through all of this.
c. After junior year: My summer after junior year will be an eventful one. I will apply for a new job
at a retail store, I am planning to make trips to Chicago with my closest friends, and also to
travel around Michigan with my family. I hope to gain the responsibility that it takes to manage
more than one event while also keeping up with my summer schoolwork.

13. Who are two Athens teachers to whom your counselor might go for further reference?
Two Athens teachers that my counselor might go to for further reference are Ms. Suleyman and
Ms. Parnell.
14. Where will you apply/have you applied to college?
I will apply to the University of Michigan, Wayne State University, University of Detroit Mercy, and
Oakland University.
15. What is your intended major?
I intend to major in Pediatric Nursing.
16. How did you become interested in this major?
I became interested in this particular major because I have always been interested in the Health Care
field but I have always wanted to work with children at the same time. Therefore, when I heard that I
could do both, it peaked my interest. I am also interested in this major because there is an average of 5
years of schooling for this degree, which is not too excessive for me. I would also be able to have a
personal life outside of work because it is not extremely time consuming.
17. What are your intended career plans?
My intended career plans is to go to college to get my masters degree as a Pediatric Nurse Practitioner
and work on the pediatric floor at Beaumont Hospital in Royal Oak. However, I would also consider
getting a masters degree being a travel nurse, to be able to travel to other countries to help those less
fortunate than I.
