SUBJECT : Required Immunizations and Medical Clearance

Please be informed that all Year Level 8 incoming medical clerks are required to submit the
following documents and laboratory results to help ensure your welfare as you transition to
the various clinical rotations:

1. Chest X-ray (AP)

2. PPD skin test
3. Anti-Hbs IgG
4. Rubeola IgG
5. Rubella IgG
6. Family physicians (or mothers) confirmation of childhood varicella, or VZV IgG

All requirements should be submitted on Monday, June 26, 2017.

The following immunizations are also highly recommended:

1. Hepatitis B vaccine: If student tests negative for anti- Hbs, three (3) doses should
be given (0, 1, 6 months)
2. MMR: If student tests negative for antibodies, two (2) doses should be giver 1
month apart
3. Meningococcal vaccine
4. Varicella: Should be given to students with no clinical history of childhood
Varicella, or VZV IgG negative.
