Your application has been electronically submitted to the Georgia Lottery Corporation. You may
wish to retain a copy of the application packet for your records. This acknowledgement of
submission of your application is not an approval of your COAM license application.

As a reminder, a business location with COAMs must prominently display an original valid
COAM location license and a copy of the appropriate valid master license.

Did not upload documents during application process?

If you were required to complete a SAVE (Systematic Alien Verification for Entitlements) form
or any other required documents (Proof of Divestiture Document, Other Required
Documents/Comments) and did not upload the required documents during the application
process, please do the following:

1. Go to the My Account Menu and click on Upload Documents and then select a Business
to proceed.

2. Click on the Choose Files/Browse button. Select the file you are uploading, and click
on Open. The file name will appear next to the Choose Files/Browse button. Click on
the Upload button to start the upload process.

If you like to upload additional files repeat Step 2.

Note: The file cannot exceed 5mb in size and should be in .jpg, .png or .pdf format.
Also, please do not attempt to upload a document that is password-protected or that
contains macros.

Your COAM license application will be in pending status until all required
documents are uploaded.
