MONTHLY SUBMISSION OF SCHOOL FORMS & DAILY TIME RECORD

To: Teaching and

All Others Concerned

1. For proper perusal and reference, the school announces the submission of Daily Time Record and
School Form 2 Monthly for checking every 1st day of every month.

2. Daily Time Record should be submitted to the school Clerk / Guidance Teacher properly filled up.

3. School Form 2 should be submitted to the School EBEIS Cordinator Mrs. Nenita C. Colasito for
properly checking. Monthly SF2 (teachers copy & for school copy) must have initial of the EBEIS
Coordinator before signing of the School Head.

4. Immediate dissemination of this memorandum is earnestly desired.
