*Liquidation Report shall be submitted on or before the 5th day of the following month
pursuant to item H.1.1. of the Manual on Simplified Accounting Guidelines &
Procedures for the Use of Non-Implementing Units/Schools, Version 2008.

The undersigned shall be responsible for the implementation and utilization of

MOOE downloaded fund with the above purpose with due diligence and efficiency
as per COA rules and regulations.
