WE, HELEN PANGANIBAN and JIMMY AYA, both residents of Mahabang Dahilig,
Batangas City, of legal age, Filipinos, after having been duly sworn to in accordance with
law hereby depose and state:

That we personally know LAMBERTO SOLOMON because we are friends and

longtime neighbors;

That he died on May 15, 2016;

That in his Social Security System record, his date of birth was entered thereon as
September 17, 1932 instead of September 17, 1931;

That his true and correct date of birth is September 17, 1931;

That his correct date of birth may be confirmed from the copy of his Certificate of
Birth hereto attached as Annex 1 and his Certificate of Death hereto attached as
Annex 2;

That this statement was made not to deceive any legal authority or to evade any
criminal and/or civil liability;

That the LAMBERTO SOLOMON, whose correct date of birth is September 17,
1931, whom we have known for a long time is the same person of LAMBERTO
SOLOMON, whose date of birth was erroneously entered as September 17, 1932 in his
records with Social Security System.
