a completed Authorship Change Form which is signed by all authors (including the author(s) being removed or
added to the paper), expressing the new author list and the reason(s) for the change in authorship before any
changes will be considered.

If you are adding an author(s), the new author(s) by signing this form agree to all conditions contained in the full
copyright transfer form supplied with initial submission.
