Scope of work
The following method statement is written to provide a safe and efficient system of work for the installation of
general working scaffold to all areas of construction sites both internal and external. It will also cover all scaffold
adaptions to general scaffold and any safety and maintenance checks to be held under current safety legislation
within the construction industry. It will cover such areas as; first installation scaffold, basic lifts (internal and
external), adaptions as work progresses during the duration of project to basic scaffolding and general working
guidelines and working practices for central operatives to strictly adhere to. Any deviation which is necessary from
M/S R/A must first be notified to Essential Scaffoldings director who will agree the best approach to M/S R/A with
principle contractors approval.

This M/S & R/S will cover the erection of scaffold staircase to crane base to allow for safe access for both crane
operative and all site personnel to lower base area which shall be incorporated into safe access for all operatives to
B1 & B2 new basement areas

NOTE: Any design scaffold which is outside the remit of this M/S R/A will require a separate M/S R/A issued to the
principle contractor for approval and agreement of time and commencement of works.

PLEASE ALSO NOTE: Only the director of Essentials can agree with principle contractor, amendments to general
M/S and must therefore be notified immediately to make arrangements for changes to be made with minimal amount
of time and cost to principle contractors project.

Sequence of work
Upon arrival to site, all Essential personnel will attend P.C. site induction, where all required site rules and
procedures and emergency procedures will be explained and strictly adhered to. Confirmation of site induction
safety paperwork will be signed by operatives to confirm induction. When this process is complete Essential site
supervisor shall then explain this M/S R/A to all operatives and explain and or answer any questions that may arise.
All operatives shall then sign this M/S R/A and a copy or the original shall be given to the P.C for their records.
Essential shall then arrange for a scaffold lorry or materials to be delivered or parked on sit- giving 24 hours
accordance with traffic management plan of site. All vehicles will be expected to park or placed in designated
loading and off loading bays behind safety barriers or contained area. All vehicles will be marshalled by Essential
supervisor, operatives, and P.C traffic marshal/banks men if available or applicable. No vehicles will be allowed
onto public footpaths foe either off loading or loading purposes without prior consent from local authority or P.C
permission (having previously gained relevant licences from local authority.) Once vehicles, off loading or loading
procedures shall commence; the first procedure shall be to install a tag line or scaffold tube the length of the
vehicle for operatives to attach safety harnesses while working at heights or to prevent falls from back of trailer. The
only time crash bars or tag lines shall be removed is if there is danger of possible clash with crane, hi.ab, or
materials getting caught causing an uneven load.

NOTE: All relevant certs for hi.ab and lorry can be produced upon request (for P.C health and safety files).

All Essential scaffolders are competent at slingers duties, but for added protection, Essential supervisor (who has a
current slingers cert) will oversee all operations with the assistance of the P.C slingers if available.
NOTE: Harness cert. and cert. for slings, straps or any equipment covered under L.O.L.E.R will be provided to P.C
for HS files and records. Also current CSCS and slingers cert. will be given at induction or emailed (which ever
required by P.C).

Essential supervisor will first inspect and check for clear access to crane base area, all and any obstructions will be
cleared by principal contractor to give a safe working area for Essential operatives, any water in base as previously
monitored must first be pumped out by others to provide a dry area to work in.
Any power cables must be cleared or placed out of working zone and made safe by others.
When Essential supervisor is satisfied with safety of crane base he will the proceed with installation of staircase but
will at all times maintain the condition of area and report immediately any issues which may cause concern.
Supervisor shall first make sure that two means of access & agrees are made available to operatives during install,
ladders shall be appropriately tied into position. Materials which have been placed as close to the crane base as
possible to reduce manual handling shall be passed to operatives inside base. (NO Bombing will be permitted).
Supervisor will make sure that all crash barrier, transport bollards and safety barriers especially on main transport

route are maintained or in position 3 party accidental access or excavation collapse, a Top Man will be in position
at all times while operatives are in box area, he will monitor for any issues and report directly to C.G.L management
if any alarms or issues are raised.
If at any time the crane lift supervisor ask operatives to leave area during lifting operations instructions will be strictly
adhered to.
Staircase will be erected in accordance with industry standards and area shall be cleared and cleared of all excess
material and equipment. Debris netting shall be erected to protect operatives when entering crane base area. Area
will then be handed to C.G.L for final inspection, then handover cert to be issued.

When all materials have been stored, the load out and erection of scaffolding can commence. Essential supervisor
will first (with C.G.L supervisor) confirm location, lift heights, lifts, structural stability, working loads and other
outstanding issues not previously raised during site visit and inspection, and to check for any unforeseen changes or
conditions which may cause concern to scaffold erection. If all issues are closed out and permission to proceed is
given, Essential will first measure and space out for standards checking at all times for ground condition,
suitability, and stability. If at this point, barriers and signage are required, C.G.L will be notified and appropriate steps
taken to minimise risk to others. When standards have been located into position, then ledgers, transoms, braces
and sway braces can begin to be installed. Standards will be placed on a minimum of a base plate and sole board if
required, however the supervisor may deem it necessary to use spreader plates or scaffold boards of greater length,
spanning multiples standards to gain stability and strength. All components of scaffolding will have under gone a
thorough inspection at Essentials main yard for inspection of defects before being sent to site. When scaffold
reaches an appropriate height there may be the need to use ring ties bolted to the walls, to give extra strength to
structure these will be placed at every 4m starting from the first anchor (including horizontally).

NOTE: At all times, 5 point PPE will be utilised:

1. Hard hat
2. Hi-Vis
3. Gloves
4. Steel toe capped boots
5. Goggles
6. Ear defenders (if required)

A harness is mandatory for all operatives and will be used and attached to suitable anchorage while working at
height or risk. Some of the team will be utilising double lanyards because of the increased risk of fall as they are
leading the installation. It may be noted that some scaffolds require ladder beams, I-beam
 For gantries or load out areas in which case, a separate addendum can be attached to this M/S R/A if the need
arises. During installation; trap doors, gates and stairs will be built into the structure. For access and protection; all
handrails, toe boards, brick guards, mid rails will be installed to industry guidelines. Monoflex or sheeting will be fire
rated to LPS1215 upon request but is latest legislation debris netting will be placed where applicable. Please note
that latest guidelines for fire regulations indicate that stair cases cannot be enclosed because of the amount of
smoke inhalation or danger to operatives during a fire. We highly recommend staircases be left open or placed out
of view if it will cause detrimental look to site. At very least has debris netting around it for free flow of air and to
prevent objects or materials falling to lower levels. When scaffold is complete and tested, P.C will be notified and a
scafftag and handover certificates will be issued. Internal guardrails to holes or excavation will be placed a minimum
of 2m from risk of fall and will be secured firmly to beams, walls, or floors toe boards will be standard. During any
work either internal or external near voids, lift shafts or anywhere an operative can be injured, Essential supervisor
will always put safety before the job and all operatives will endeavour to make all installations and strip down
procedures as safe and secure as reasonably practical. A simple handrail will require an operative to attach himself
to a secure point at all times. If not available, Essential supervisor to inform P.C immediately. Scaffold lay downs
may require Essential to install floor ring ties during the instillation for operative safety if this cannot be achieved
then A frames may be required or wall or window ties. If this is still unachievable Essential supervisor will speak
directly to C.G.L to achieve best way forward.

Please note that installation of basic scaffolding internal / external is a very generis process. Top rails, mid rails, toe
boards, walkways, ladder heights, double couplings, spacing of standards, ledgers, transoms, heights of walkways,
sway bracing and so on basic will all be completed in accordance with current industry standards; i.e. TG20 and
also, any P.Cs site requirements.

Please note: This is an amendment to general M/S and should only be used in conjunction with the original that can
be found in C.G.L health and safety files.

Monitoring

Site management, Essential H&S representative, visiting managers and all safety advisors will monitor personnel
compliance and working practices for any unsafe acts or working conditions. Weekly scaffold inspections, safety
documentation to be issued weekly to P.C.

Site hours 08.00-18.00,mon-fri, duration TBC, weekend working in agreement with C.G.L

Hazard Identification and Risk Assessment

Because of the nature of Essential Scaffoldings work some risks will always be prevalent, these are:
1. Working at height R/A attached
2. Slips, trips, and falls R/A attached

5. Cuts and bruises R/A attached
6. Wearing of safety harness R/A attached
Because of the nature of this particular project we foresee these additional risks

NOTE: As the nature of the job progresses certain risks may increase or decrease. M/S R/A will be amended in line
with project at regular monitored levels or upon request from P.C. All additional R/A will be included.

Preventative and control measures

All hazards identified above will be controlled by M/S R/A but in addition, Essential supervisor will constantly monitor
site conditions for any deviation from the M/S R/A which may be required and notify the P.C immediately and then
further contact Essentials director. Once M/S R/A are implemented fully, then all risks are dramatically reduced as
far as is reasonably practical.

Training certification
All Essential scaffolders have current CSCS & CISRS; in addition, Paul Morgan shall be the trained first aider and
has also received training on crane rescue techniques. At least 3 operatives have asbestos awareness training and
also working at heights and harness training. Paul Morgan has also received CISRS advanced scaffold inspection
training. All certification is attached or available on request.

The nature of work will require the number of operatives to increase and decrease as work progresses but named
personnel above will be responsible in the first instance for implementation and protocol on all sites.

Plant & Resource

Essential scaffolding has 1x open back lorry, heavy duty lorry with Hi-Ab fully certified, insured and MOT. Current
certification upon request.

Any scaffold lifts required on site by crane or genie or scissor lift will be implemented by P.C or by instruction only to
Essentials.

Various scaffold hand tools will be utilized; i.e. spanners, tape measures, levels, reciprocating saws. Any specialist
equipment P.C to be notified.

Access and Egress

All Essentials scaffolders shall stick ideally to P.Cs main access and egress routes as agreed and stated during site
induction. Any deviation which is required shall be in agreement with P.C and agreed route implemented and
controlled to minimise third party injuries, accidents, or collisions.
Two ladders are to be utilized to give and maintain safe access to crane pit, access shall be by main site access in
transport area to base.

Waste control
Essential are fully aware of C.G.L. site waste management plan and all arising waste resulting from Essentials work
shall be cleared as work progresses to either designated areas or designated waste bins or skips as per P.C
instructions. All areas where Essentials operatives have worked will be thoroughly inspected and cleared of any
debris or waste.

Third party protection

Refer to M/SR/A for controlled measures while working with and around others. In addition, Essential supervisor
shall constantly monitor or work in progress for any possible incidents which could arise if he feels he cannot
safely continue work without the risk of injury to his operatives or others, he will stop work, notify P.C. until an
agreed approach is implemented with no risk or as reasonably practicable. All barriers, signage, controlled areas
will either be implemented or be maintained and replaced if disturbance is necessary- P.C. to be notified in this
instance
 Emergency Arrangements
All emergency arrangements explained to Essential operatives during induction shall be fully implemented without
deviation as far as reasonable practicable. Essential supervisor shall make sure that all operatives know the location
of the First Aid point, Muster point, nearest hospital and what to do in the event of an operative having incidents
causing the situation of a deployed harness (see below for brief description). They are also aware of the sensitivity
of the area and the risk of a bomb threat- should this need arise, all operatives will be told by P.C. to gather at a
designated area.

Rescue plan

Rescue: (refer to flow process SG4.10 and SG20)

The rescue plan for this job entails the use of short rope lanyards. In the event of a fall the lanyard will
prevent the scaffolder from falling below the working steels. This will allow him to rescue himself with
assistance from his colleagues. The scaffolders also have last resort harness stirrups; these can be
deployed in the event of a fall allowing the scaffolder 15 mins extra time before harness trauma will set in.
Once rescued the operative will be fully assessed by a trained first aider or a member of the emergency
services. Any such incident should be reported to a manager immediately and recorded in the accident
book. After the incident a full investigation will be carried out as to why this occurred by the scaffolding
management team and a written report supplied to main contractor. The harness and lanyard will discarded
for full inspection immediately after the incident and a new one provided by the scaffolding contractor.

SG04:10 show a case study of a scaffolder that fell on an independent tied scaffold. His colleagues were
able to facilitate a rescue by moving platform boards to a lowered lift to improvise a temporary platform at
the same level as the casualty. He was pulled into the safe platform until the emergency services arrived. In
the mean time first aid was administered and safe access to the lift provided.

Principal Contractor to provide scaffold contractor with the nearest Accident and Emergency Hospital as
per their Health and Safety Plan

WE STRONGLY RECOMEND THE EMERGANCY SERVICES ARE FIRST TO PROVIDE CARE TO OUR
OPPERATIVES ALTHOUGH IN SOME CASES FULLY TRAINNED FIRST AIDERS MAY NEED TO ACT.

Communication
Once commencement of work has been approved, Essentials supervisor (Paul Morgan) will be responsible in
implementing safe working procedures. All Essentials operatives will be required to read and sign M/S R/A and to
further sign C.G.L CS06 for proof of task briefing. These will be held on record in C.G.L H&S files for inspection and
amendments as and when the need arises. Further, all Essential operatives will be required to attend weekly toolbox
talks, held by C.G.L for updates to site condition or legislation and to attend Essential Scaffoldings internal tool box
talks both will require CS06 for proof of attendance.

NOTE: Essential understands that it is of upmost importance to attend toolbox talks to make sure all operatives are
kept up to date with site changes which will occur rapidly. An example would be the installation of a tower crane or
exclusion zones.

PPE
Chorus requirement on all their sites of 5 point PPE will be fully implemented without exception. No Essential
operative has any authority to de- regulate this requirement. Breeches may incur internal disciplinary procedures or
Chorus issuing yellow or red cards.

Mandatory PPE:

5. Goggles suitable for task
It will also be mandatory to wear a suitable harness and lanyard of type and design will be dictated by task. Inertia or
tag lines may also be required for task; i.e. off loading the lorry.
All equipment will have relevant certification to be provided to Chorus for their L.O.L.E.R and H&S files. Further, it
may be necessary for our operatives to wear dust masks (FP3 minimum) or hearing defenders. See attached R/A
for noise or protective overalls.

Essential supervisor to monitor and implement.

Power Supply
Chorus to supply site temps of 110v rating. All Essential power tools i.e. receipt saw will be; 110v, pat tested every 3
months and in accordance with latest industry guidelines. Any further requirements; i.e.415 3 phase will be in
agreement with P.C and will require an addendum to M/S R/A for implementation. Site 10KVA supply units will be
utilised but as work progresses; smaller, isolated 3.3KVA may be needed with RCD protection.

Lighting
Chorus site temps to light all stairways, landings, walkways, and emergency escapes, any task lighting will be
supplied by Essential. We understand C.G.L requirements on non halogen lights therefore sodium will be supplied
if required.

Working Platforms

Because of the nature of Essentials work, all working platforms are built as work progresses, i.e. ground up. But it
may be necessary to utilise at times a mobile tower, therefore, only a competent person will erect mobile towers.
A scafftag will be mandatory. It may also be necessary for a MEWP to be utilised for the erecting or dismantling
process. If this is necessary, then a separate R/A will be issued under supervision and agreement with P.C.

Excavations and Temporary Works

See attached R/A
Fire

None of the work implemented by Essential will create heat, sparks or flames. If Essential supervisor feels this will
change then a separate R/A will be issued for task, so work may commence under the monitoring of C.G.L with a
hot works permit in place.

In the event of a fire, all Essential operatives will as instructed by Chorus fire marshal be required to go to
designated muster point and to follow instructions given by C.G.L. All standard evacuating procedures will be strictly
adhered to.

Information and Inspection

This M/S R/A will be submitted to C.G.L for their approval and comment. Once approved, M/S R/A will be fully
implemented by Essential. All relevant safety paper work regarding working at height will be followed and
implemented.

Weekly scaffold inspection will take place and supporting documentation to be issued to P.C for H&S files and
comments.

Erecting Procedure:
Base out scaffold for light duties and foot traffic

The lift will be positioned at the required height. Note: this will be agreed on site between essentials supervisor
and C.G.L.
Erect standards on top of sole boards and base plates to working area.
Ledgers can then be fixed using double couplings. These will be spaced in agreement with C.G.L and essentials
supervisor on site.
This process will be continued along the length of the projected area. Once this is completed transoms can be
fixed again using double couplings ensuring to support the boards every 1.2m. The minimum and maximum
board overhangs will be observed (Min 50mm, max 150mm).
This can now be fully boarded to minimise the risk of falling plus a handrail and a nonworking lift will be accord-
ing to SG4.10

2 and final lift

Ledgers to be sized so that joints are staggered and do not fall in the same bay. Ledgers to be fixed to the stan-
dards using right-angle couplers and must be level in accordance with the tolerances stated in TG20:08. Joints in
Ledgers will be made using sleeve couplers only.
The lift will be positioned at the required height. Note: this will be agreed on site between essentials supervisor
and C.G.L.

Fix 1 lift transoms will be fixed to the ledgers with putlog clips adjacent to the standards and to support the
boards every 1.2m. The minimum and maximum board overhangs will be observed (Min 50mm, max 150mm).
This lift will be fully boarded.
Scaffolders will use the Appendix A principle by installing guardrails progressively from below to avoid the need
to access unprotected lift.
Working away from the ladder position scaffolders will fix bottom guardrails progressively to the standards using
right angle couplers. The main guardrail will be fixed at a minimum height of 950mm above the working platform.
An intermediate guardrail will be fixed to ensure no vertical gap greater than 470mm between guardrails.
Once all guardrails are co
Complete (including stop-ends) toe boards will be fitted to all external elevations and the window reveals. All to
be boards will be secured at a minimum of two positions using putlog clips to the standards. Stop-ends will be
fixed using a dead-man puncheon where necessary.
Scaffolders will use the advanced guardrail principle by installing guardrails progressively from below using step
system.
Ledger bracing to fixed to the end pairs of standards and at alternative pairs of standards, using swivel couplers
within 300mm of the node point.
Faade bracing to be fixed to the standards close to the ground and also within 300mm of the node point, using
swivel couplers. A face brace shall be fixed to 1 in 6 bays and may be over two bays as per TG20:08.
Once the scaffold has been erected essentials supervisor will offer to C.G.L for inspection, along with handover
certificate and Scaff tag.

Staircase

. 8ft Rake. Fix staircase ledgers to road level from top boarded lift of scaffold using right angled coupler.
An Aberdeen transom will be fitted in the middle of the ledgers using right angled couplers.
Spur-Braces shall then be fitted using swivel couples on the standards and right angled couplers on the transoms.
Starting from the bottom more temporary treads can be fixed with doubles. This will allow a safe working point
from which to fix permanent treads.
Stair treads will then be fitted using right angled couplers at a set spacing agreed by the site manager and

Staircase:

If intermediate standards are required a 5ft tube can be under slung on doubles fixing also to the standards.
Scaffolders will fix bottom guardrails progressively to the standards using right angle couplers. The main guard-
rail will be fixed at a minimum height of 950mm above the working platform. An intermediate guardrail will be
fixed to ensure no vertical gap greater than 470mm between guardrails.
Once all guardrails are complete (including stop-ends) toe boards will be fitted to all external elevations and the
window reveals. All toe boards will be secured at a minimum of two positions using putlog clips to the standards.
Stop-ends will be fixed using a dead-man puncheon where necessary.
The staircase up to the first loading platform is now complete and shall be repeated on the reverse side to the
top lift.

Dismantle Phase is often reverse procedure of erection but hazards may change

A of SG4:05 (step system) advanced guardrail.

All scaffolding will be loaded onto the wagon and taken away that day.

Adaptation Phase

If the scaffold is to be adapted then if the scaffolders are not onsite then it will be a maximum of 48hours call off.
