Subject: Account Discrepancy

To whom it may concern,

Good day. I am writing to your good office for the reason that there is a discrepancy on
my bank account under the name of James Ryan Delos Santos with account number 125-
36100591-3.

I would like to ask for some help because Im really disappointed right now with regards
to this issue. My last withdrawal was on June 13, 2017 with an amount of Php 2,000.00 in
PSBANK-Binan Branch.

Last Monday, June 19, 2017, I deposited Php 12,000.00 at PSBANK Alabang-Zapote
Branch and updated my passbook. I was surprised that theres a transaction withdrawal amounting
Php 2,495.85 last June 18,2017 via POS or with transaction code TR. I only connected my account
to my apple id recently and Im quite sure that I didnt do any transaction within that day. I even
checked my Apple account if theres an unauthorized transaction, but theres none.

I am looking forward to your immediate response and action with this matter as I put my
trust in your well esteemed company.
