1.0 INTRODUCTION
Paediatric physiotherapy is the treatment, by physical means, of conditions
seen in children, which affect or compromise normal childhood development. The
paediatric community Physiotherapy services provides specialist assessment
and a range of interventions to enable the children with physical difficulties to
reach their full potential by maximising function and independence and promoting
normal movement; aiming to prevent limit contractures and deformity, and
thereby improving quality of life.

The physiotherapist teach the families and carers about the child presents
with the programme of activities required to either maintain or to develop skills.
Partnership with parents and education staff is essential.

Chronic pain can be so disabling that it can prevent a child from participating
or join other children or family doing normal activities in their age range such as
doing outdoor activities, going to school, join the social events, and sports that
they like. It also can lead to isolation and also some got in depression as the
child has to grow up with abnormalities with their family and friends.

When a child facing the worse condition and situation, their family also
involved to their problem. Instead the parents are trying finding ways to help their
child, in the same time they also facing the stress of the work and also other
disruptions of daily living.

So this one stop paediatrics rehabilitation centre can help the parents to solve
their child problem. We carry the hope that those children and families can
coping with the chronic pain and related functional disability, even after standard
therapies, surgeries and medication have been exhausted. This rehabilitation
centre are built to help the children and also the families to reclaim their lives
same as other normal families.

This rehabilitation department provide services to children who have
difficulties with gross motor skills including delayed development, balance and
co-ordination. At this centre, all therapies work together in treating acute and
chronic of respiratory conditions, elective orthopaedic injuries and some neuro-
rehabilitation cases. This centre also will help to treat the premature babies
including the babies who are born with Cerebral Palsy and Brachial Plexus Palsy.

At our paediatric treatment room will be open only on weekdays which is from
Monday till Friday. This department will be opened at 10a.m. 5p.m. and the
registration counter will be opened starting on 9.30 a.m. during morning session
and on 2 p.m. for evening session.

This department also provide the experiences therapies to treat the children.
Total of our therapies are five which consist of the two experience
physiotherapies and also work as physiotherapys lecturers and three
experiences and skilful alumni Physiotherapy students from University Kuala
Lumpur Royal College of Medicine Perak. This therapies will work according to
the shift which is 3 on the morning till 1p.m. on the lunch shift and 2 will be
replaced on evening session that is from 2 p.m. till 5.p.m. Our department also
open the chances to physiotherapy students that want to gain the experiences
and skill at our paediatrics treatment room but limit only to 3 students for a
month. We opened to all medical college that have interested to let their students
to do the clinical posting at our department.

2.0 TITLE
Proposing Twinkle Physiotherapy Centre

3.0 RATIONALE
The children that face the health problem should get the proper treatment to
help them continue their live same as other normal children. These kind of
children can be help by the physiotherapist that actually work with people of
varying ages from premature babies to adolescents to ensure optimal physical
function and development. We are very concerned with movement, co-

ordination, body posture. Also, in paediatric cases is to provide a program that
the children and families will enjoy and encourage them to participate with other
normal children and become independent in daily lives.

The aims of physiotherapist is to minimise the effects of physical impairment


to promote optimum function and musculoskeletal development. Advice to
parents and children on activities and stretches offered by the physiotherapists
can be assist by maintaining full range of movement and prevention of
contracture.

Benefits of get assess and treat infants and children with a range of conditions
such as Cerebral Palsy (from mild hemiplegia to severe quadriplegia),
Developmental delay (due to hypotonia with/without diagnosis and may be gross
motor or global), Posture problems (torticollis, scoliosis, talipes, metatarsus
adductus), repiratory problems such as Cystic Fibrosis or asthma and more
others.
The physiotherapist in paediatric is to assess the referred child and give parents
and/or carers advice regarding handling, positioning and treatment through play
and/or exercise. Physiotherapist work closely with families, carers, teachers and
doctor and other health professionals. The approach is holistic and practical, with
an emphasis on gross motor function and posture. For better outcomes and most
effective treatment results, early referral is the key (before eight months). Infants
and children can be seen at home, day care centre, early intervention programs,
schools or clinics on a regular basis.

Advice will be given on appropriate handling and equipment including methods


may be used such as neurodevelopmental therapy, motor learning and
hydrotherapy. Physiotherapist are often the firs therapist to see the child amd are
well received by parents.
Paediatric physiotherapist also work with a range of conditions to help older
children and adolescents, including:
Acquired brain injury and spinal injury
Neurological diseases
Post trauma injuries, such as fractures, sport injuries, post orthopaedic
surgery

Burn and plastic surgery
Limb deficiency conditions
Chronic pain
Fractures
Musculoskeletal conditions such as soft tissues injury, back pain,knee
pain, postural problems, and also joint problems
Neurodevelopmental problems
Neuromuscular conditions

Physiotherapist role as help to maintain and develop functions skill level and range
of movement in order to minimise joint contracture and postural deformities. They
encourage children to partake in wide range of activities and communities to
maintain physical fitness and provide opportunities for socialisation with their peers.
They also prescribe and monitor the use of aids such as orthotics, walking aids, and
wheelchair to maintain independence.

from hospitals or clinic
The assessment and provision of Orthothics for the children and young
people that we can see with some of neurodevelopmental and
neuromuscular conditions to facilitate improved gait patterns and
minimise deformity
Assessment and ordering of mobility and postural support equipment.

4.0 PURPOSE
The purpose of this Paediatric Department are:
To provide the specialist assessment, intervention, advice and guidance
for children and their families where the delay is attaining the childrens
gross motor skills such as late sitting or walking due to variety of reasons
Also to minimise physical disabilities and maximise physical abilities and
independence of our patient.
Will work as part of the multi-agency team that providing our support and
education on physiotherapy issues to our patients, their parents and/or
families.
Provides rehabilitation services to children in aged 0-12 years that having
neurological - developmental conditions such as cerebral palsy, muscular
dystrophy, spinal bifida, neurological syndromes, chromosome disorders,
developmental delay, toe walking, gait anomalies and children with
cardiorespiratory problems.

We also has provided the spinal physiotherapist who will give the best
treatment to patients with range of spinal deformities that been referred by
our spinal consultants.

At this paediatric department, we as the physiotherapist will provide both


elective (planned) and non-elective (emergency) inpatient care to infants,
children and young people such as:

Outpatient services
Children assessment unit, where the children are first seen and
assessed when referred by doctors of other hospitals or nearest
clinics.
Ambulatory care for planned care, including specialist immunisation
and vaccinations.
Treatment and care for children that involved with surgical condition.
What parents can expect from us as the therapist are their children will be
offered an appointment at our centre. One of our physiotherapist will assess their
child and planning the next treatment together with the parents depending on
their child needs.

As the number of patients will be precise, we divided the patient for musculoskeletal
case and neurological case for a day and during weekend.

6. VENUE

Our aim is to make patients easy to find our centre. Paediatrics centre is located at
second floor, Manual room of physiotherapy, New building of UniKL Royal College of
Medicine Perak, 3, Jalan Greentown, 30450 Ipoh, Perak, as it is well-known campus.
It near to the town that have lots of facilities and accommodation such as:

Angsana Mall

Hospital Raja Permaisuri Bainun

Masjid Hospital Ipoh

KWSP

Angsana mall and KWSP was opposite to the main campus of UniKL Royal College
of Medicine Perak while Hospital Raja Permaisuri Bainun and Masjid Hospital Ipoh at
the back of campus. Many facilities such as parking was also provided. The place we
choose is strategic and scenery is beautiful and not so much hectic that patient will
satisfy.

11. CONCLUSION

The conclusion is children that facing the health problem or paediatric problem
usually has the development or growth problem. Their daily life activity also limited
not like the others normal children. Physiotherapies treatment very helpful in
improving resolving or reduce problem in paediatric cases. Finding the good
rehabilitation centre is important, that is the reason why we proposing to open the
Twinkle Centre to help parents in treating their children without puts any limitation.

Location we choose are strategic and easy for patients to find the exact place. It
is near to the town that many facilities is provided. Such as, parking place, Angsana
Mall, Hospital Raja Permaisuri Bainun and also got KWSP. Patients are also can find
beautiful scenery and can gain calmness as it is not hectic place. So, we can
conclude that the place we choose is right and suitable for our patients.

Also, our centre will operate on weekdays as well as Saturday so that parents
who are busy can spend time sending their children to the centre. The employees
involved in this centre are therapists which is lecturers, therapist assistants
consisting of students and alumni, cleaner and receptionist.

Based on our floor plan, our center will provide enough equipment and suit for
neurology and musculoskeletal cases. We also provide 3 fire extinguisher for the
safety of children and parents. Besides, we provide television as an additional
attraction to attract the attention of children during the treatment.

We try to minimize the budget will be used for Twinkle Physiotherapy Centre. We
chose the best price from our searching. For decorations, we thought to use RM4
761.00. While, for equipment RM3 210.00. Interior designs and advertising are from
RM5 000.00 and RM770.00. So, for the grand total is RM13 741.00.

Last but not least, we want to maintain a friendly, fair, and creative work service,
which respects diversity, ideas, and hard work. Therefore, we wish we could carry
the hope that those children and families can coping with the chronic pain and
related functional disability, even after standard therapies, surgeries and medication
have been exhausted. This rehabilitation centre are built to help the children and
also the families to reclaim their lives same as other normal families
