Reason for Referral

Jaces parents are concerned about the milestones he has missed, particularly relating to verbal
communication. Theyre also very concerned about Jace not making eye contact with them.
Based on these concerns theyre worried that Jace may have a disability.

Background

Developmental History

While pregnant with Jace, his mom had a typical pregnancy and delivery with no complications,
and tests after birth went well. His gross motor skills, such as rolling over, crawling, and walking
developed fairly early which is typical. However, his language skills were delayed. At 6 months
old, Jaces parents noticed that he wouldnt make eye contact and by 15 months old, his
parents began to worry because he had only babbled a little and wasnt saying any words. Both
are considered atypical development for those ages. After Jace received tubes in his ears at 18
months, he began to babble a little more. A few months later, Jaces parents saw a girl on the
news who had autism, which they had never heard of until then. They recognized that the girl
had similar developmental delays as Jace, so they contacted their local universitys Infant and
Toddler program. Jace currently doesnt mimic others actions, point to things, jump, or throw a
ball.

Family Information and Social/Emotional History

Jace comes from a loving, supportive family. His father attends school at the local university
while his mom stays at home with Jace and his 3-month-old sister. They live within two hours
from Jaces grandparents and visit them often to give Jace opportunities to play outside more.
Jace gets excited when he sees his parents and often looks to them before doing something
theyre doing, such as waving goodbye or folding his arms. Jace doesnt imitate his parents
facial expressions or understand the concept of emotions. Jace mostly ignores his baby sister
unless she is upset or crying, then he will pull his mother over to his sister and place her hand
on the baby. He started attending nursery at 16 months two months earlier than most
children and separates well from his parents, although he doesnt interact with the other
children.

Medical

Jace is fully immunized, and those immunizations are up to date. Hes been prescribed steroids
for croup and will still get a bad cough when he gets sick. He hasnt had his vision tested but has
had his hearing tested and can hear out of both ears. He has had multiple ear infections starting
at age 6 months, and his parents took him to a hearing and balance center as well as an ear
specialist. At 18 months, he received tubes in his ears from fluid buildup. In the past, his
mothers brother also had to get tubes in his ears. Jaces pediatrician is aware of his language
delay and suggested reading word books with him and to return at 2 years old for further
examination.

Educational History

Jace has no current education history, but his parents have applied for him to attend toddler
lab at R Child Labs.
