Directions: Please note candidate’s strengths or areas in which s/he can improve in each category
below. When possible, please provide specific examples or comments to support your view.

1. Conducts self in a professional manner (e.g., attendance, preparation, respect)

Kelly was a pleasure to have volunteering with us on Thursday nights. She was reliable and
did a great job of being actively involved with the participants. She always came prepared
and had a positive attitude.

2. Encourages development of a positive self-image in learners e.g., gets to know students,

positive interactions with students)
Great job interacting with the participants and getting to know them. Socialization is a large
part of what we do during our activities and she did a great job engaging everyone in
conversations.
3. Interacts effectively with students, colleagues, administrators, parents
Had positive and effective interactions with all students, staff, and supervisors in all of my
observations of her.
4. Demonstrates effective pre-professional teaching skills (e.g., planning, implementing
lesson)
Not a skill I observed, as we did not require her to develop lessons or implement them. Was
always willing to help out with the activities.

5. Communicates effectively through speaking, writing, and listening modes (e.g., gives clear
directions, listens to students, uses media)
Kelly communicated effectively with all involved in the program. She was patient, helpful, and
encouraging to all.

Final Comments/Suggestions for Candidate:
