no MIDDLE NAME;

3. That we are not related in any way, either by consanguinity or affinity to CHRISTINE
APRIL CREER;

4. I am executing this Affidavit to attest to the truth of the foregoing and to correct my
data in my certificate and file records of the Civil Service Commission.
