We, MARICHELLE B. GARCIA and DIANA ROSE M. CONISANG, both of legal
age, Filipinos, with residence and postal address at MacArthur Highway, Brgy. Talaga, Capas
2315, Tarlac, Philippines, after having been duly sworn to in accordance with law, do hereby
depose and say:

THAT—

1. We are the attorneys-in-fact of the heirs of the late JUANITO B. SANTOS and
RAMON B. SANTOS, a copy of the Special Power of Attorney is attached as
Annex “A” to form an integral part hereof;

2. We know that the late JUANITO B. SANTOS and RAMON B. SANTOS died on
October 12, 2012 & ___________________ at Mt. View Subdivision,
Brgy. O`Donnell, Capas, Tarlac, Philippines &
____________________________________ respectively;

3. We also know that the fact of death of JUANITO B. SANTOS and RAMON B.
SANTOS were not registered with the Local Civil Registry of Capas, Tarlac and
the Philippine Statistics Authority, Manila, due to inadvertence;

4. We are executing this Joint Affidavit to attest to the truth of all the foregoing; to
attest under oath that JUANITO B. SANTOS and RAMON B. SANTOS died on
