AFFIDAVIT OF WAIVER
I, Jaira S. Nobleza, 23 years old, student of City of Malabon University and
residing at No 195 Sanapu Hoa Sitio 6 Malabon City, after having been dully sworn
to, in accordance with the law, do hereby depose and states THAT;
I am currently engaged in On the Job Training (OJT) at Philippine National
Police (PNP) as part of the academic requirements of my course on, B.S
CRIMINOLOGY 4A – City of Malabon University.

The entire OJT program comprises for the First Trimester is 270 hours.

My tour of duty in the program is scheduled at (8:00AM to 5:00PM) daily, within

the premises of Philippine National Police Malabon.

In line with the above foregoing, I hereby voluntary waive my rights against the
school (City of Malabon University) for any claim or whatsoever should any untoward
incident occurred while I am on my OJT program and conducting affairs outside the
school premises or at the premises of Philippine National Police Malabon.

I hereby declare that the school has no obligation on any atrocious incident which
could be done beyond its control and supervision.

That I am executing this affidavit to give effect my express waiver of rights for the
above rights mentioned and to attest the truthfulness of the above statement;
