KNOW ALL MEN BY THESE PRESENTS:

This Deed of Extra-Judicial Settlement of Estate is made and entered into by and among:

xxxxxxxxxxxxxxx, of legal age, single, Filipino, and a resident of

xxxxxxxxxxxxxxx, of legal age, single, Filipino, and a resident of

(All of which are hereinafter referred to as the “HEIRS”)

WITNESETH THAT:

WHEREAS, yyyyyyyyyyyyyyy (hereinafter referred to as the “DECEDENT”) died on

________________ in the City of Tacloban without any will or testament and without any
outstanding debt in favor of any person or entity. A copy of the death certificate of the
DECEDENT is herein attached as Annex “A”;

WHEREAS, the abovenamed stated HEIRS are the legal heirs of the DECEDENT by
reason of the fact that they are her full-blood children supposed to inherit by way of intestate
succession;

WHEREAS, at the time of death of the DECEDENT, she had a joint bank account with
one of the HEIRS, namely, _____________________, and she owns half of the deposits of
money at LandBank of the Philippines – ABC Branch, more particularly described as
follows:

NOW THEREFORE, for and in consideration of the foregoing premises and invoking the
provision of Sec.1, Rule 74 of the Rules of Court, the said HEIRS have agreed to settle the and
adjudicate the said personal properties/cash deposits extrajudicially. And to this effect, the
undersigned HEIRS do hereby partition and adjudicate the same unto themselves in equal shares

the foregoing inheritance particularly the cash deposits at LandBank of the Philippines –
Catbalogan Branch, subject to the liabilities imposed upon by Section 4 of the same Rule.
