1. Employee Facilities Score: Serious 3 C

Lunchroom area in back of the kitchen all equipments needed to be cleaned, inside and out. Most class rooms were dusty and clutter. Window sills had dead flies. Milk
crates needed to be removed in most rooms.

2. Food Tempuratures Score: Critical A

Temp. logs were all up to date. No issues with the any of the paperwork.

3. Food Storage & Handling Score: Critical 3 C

Freezers and Coolers in back of house were dirty and needed to be organized. The dates needed to be complete or use labels.

4. Dry Storage Score: Serious 1 B

Small area, storage were outside the kitchen area. The staff made do with the space that they had.

5. Food Service Equipment & Vending Machines Score: Serious 3 C

Most equipment in back of house needed to be cleaned. .Coolers, freezers, stoves, drawers, and full dates on items.

Utensils was ok, The front of the house equipment was cleaned. The serving line was cleaned.

15. Ceilings & Walls Score: Critical C

Most walls, baseboards thur out building needs to be painted. In several rooms and halls peeling paint was noticed. This should be corrected as soon as possible. The
auditorium was very dusty and a walls was peeling very bad, the projector room was dirty and cluster needs to be removed and walls needed painting. The ceiling in
halls also had peeling paint.

All baseboards thru out building needs to be painted, some cracks noticed near entrance doors.

Restrooms were clean. Water were hot. The toliet paper is kept in office.

18. Restrooms Score: Critical B

Restrooms were clean. Water were hot. I saw no issues.

19. Basement / Engineering Facilities Score: Critical D

Mech.Room was filthy, needs more cleaning, the basment area needs more cleaning and clutter remove. Fridge was noticed in engineers office. The engineer was told to
make sure he takes the Boiler rooms chemical reading. Mice dropping notice in basement area. The boiler room also needs to be cleaned. The egineer also was told to
remove tape off sprinkle heads.

20. Safety Score: Serious B

Fire extingishers were not noted that they had been checked monthly. The kitchen staff and custodian staff were wearing slip resistant shoes. There were
mats at all entrances to prevent anyone entering the building from falling. Doors were all locked, you had to be let in .Wet floor signs were noticed.
