Re: CCS Oncology

Dear CCS Oncology Patient:

On March 2L,20L8, a decision and order was entered in Bank of America vs Won Sam Yi, et
al. (17-CV-01283EAWJ. In anticipation of the execution of an order of seizure in favor of
Bank of America, the government is taking steps to secure information in an ongoing
investigation.

The government's actions will result in the copying of certain information in the custody of
CCS Oncology. Your medical information will remain with CCS Oncology.

If you need to discuss continuation of the care that you have been receiving at CCS
Oncology, we recommend that you speak immediately to your primary care physician. If
you do not have a primary care physician or you do not believe that speaking to him or her
is an appropriate option for you, attached is a list of area hematology/oncology centers
along with their contact information. This list is not exhaustive and the inclusion of an
institution on this list does not represent the government's endorsement of care at that
institution. You should, of course, explore whatever options and make the treatment
decisions that are appropriate for you.
