10 RIGHTS:

 Identifies the RIGHT REASON for the medication as it pertains to the pt

 Identifies the RIGHT MEDICATION

 Calculates and gives the RIGHT DOSE

 Identifies and gives medication at the RIGHT TIME AND FREQUENCY

 Prepares the medication using the RIGHT ROUTE

 Performs RIGHT PT EDUCATION (explains procedure & purpose of medication to pt)

 Identifies RIGHT PATIENT by comparing the wrist band against the MAR AND using at least 2 identifiers (name,
hospital ID #, birthdate). Asks client to state name and birthdate

 Students performs the RIGHT DOCUMENTATION on MAR and other records correctly

 Student performs the RIGHT EVALUATION (ie. Was med effective, adverse events)

 Student allows the client the RIGHT TO REFUSE the medication.

3 CHECKS:

 Checks the medication against the MAR when taking it from the Med drawer (1 st check).

 Checks the medication against the MAR a second time (ex. when taking it from its container or putting it in a
med cup or syringe) (2nd check)

 Checks the medication against MAR when about to administer it at the bedside (3 rd check)
