Snellen Chart for Adults

Instructions for use:

1. Print this PDF on an A4 size sheet of paper.
2. Tape or pin the chart at eye level to a bare
wall in a room with no windows.
3. Standing at a distance of 3 meters, have a
friend or family member shine a light in the
direction of the chart.
4. While covering 1 eye, read the letters on the
chart out loud. Continue to the bottom line,
or until the letters are too difficult to read.
5. Record the numbers next to the smallest line
in which you can correctly read the majority
of the letters.
6. Cover the other eye, and repeat steps 4 and 5.
7. Adults with healthy vision should be able
to clearly read the 20/20 line. If you
cannot,you should seek an evaluation by
an eye care professional.

Remember, this test is not a diagnostic tool, and cannot be used as a substitute for professional medical opinion.
