AFFIDAVIT OF TRANSFER OF BUSINESS NAME

I, TEODOMERO M. DESTORA, Filipino, of legal age, married and

a resident of Totolan, Dauis, Bohol, after being sworn, depose and say:

1. That I am the registered owner of Teodie House Rental as evidenced by

a DTI Certificate with no. 05084202;

2. That I am transferring the said business to my wife, JANE DESTORA,

as she will now be taking over and will be managing the said business;

3. That this transfer involves solely the business name of the

establishment only and does not include the properties thereto;

4. That I execute this affidavit as to testify the truth of the foregoing and
as well as for any other legal purpose this may serve.
