Construction Safety Hazard Analysis

Workers reinforcing a concrete staircase with steel bars face several hazards. Carrying rebar manually could cause strains and cuts from sharp edges. Working at heights without fall protection poses a risk of falling. Using an oxy-acetylene torch to cut steel presents fire and burn hazards. Sharp tie wires and manual rebar bending can also cause injuries. Proper lifting techniques, fall protection, fire safety measures, protective equipment, tools, clothing, hydration, and shaded working areas are needed to control these risks.

JOB LOCATION: ANALYST: DATE:

04/30/18

TASK DESCRIPTION:
Workers climb in between the
ground floor and second floor to
reinforce the concrete staircase
with steel bars. One worker grasps
a rebar and bend it by hands using
manual bender. The other worker
was using Oxy Acetylene Torch in
cutting steel.

HAZARD DESCRIPTION:
Carrying steel up to the second floor manually and sorting
them could result in strains, sprains and fractures, rebars
edges are sharp and could result in cuts or steel splinters to
hands or arms.

HAZARD CONTROLS:
1. Unload rebar from trailers as close as possible to work area.
Use mechanical lifting devices whenever possible.
2. Do not overextend. Do not carry more than you can handle.
Take regular breaks to relieve fatigue from constant lifting.
3. Always wear the required Personal Protective Equipment, e.g.
eye protection, sleeved shirts, long pants, suitably strong and
flexible leather gloves, and sturdy boots.

HAZARD DESCRIPTION:
Working near at the edge of the staircase could result to
falling from the edge of the work area. Walking also over
steel could result to cuts, slip on steel, fall; trip, twist ankle
or knee.

HAZARD CONTROLS:
1. Place handrail, midrail, or guardrail. Adequate perimeter edge
protection is required.
2. Make sure that only trained workers are climbing and working
in high spots.
3. Be mindful and watch your feet while walking.
4. Penetrations must have secured, covered, and marked.
5. Wear safety harness and install safety nets. Sturdy boots are
required to protect your feet from steel.

Cutting steel with Oxy Acetylene torch could result in fire
and/or burns to the body e.g. arms and legs, and damage to
eyes.

HAZARD CONTROLS:
1. Remove nearby combustible materials (such as flammable
liquids, wood, paper, textiles, packaging or plastics);
2. Evaluate the particulars of the work, such as duration and
frequency of the cutting operation.
3. Provide training and use only qualified employees to do cutting
operations.
4. Provide personal protective equipment to the workers
concerned against the hazards during the cutting process.
5. Use eye and face protection. Although a welding hood may not
be use, a face shield with appropriate UV protection is needed.

HAZARD DESCRIPTION:
Sharp edges on tie wire that are used to secure rebars could
result in cuts or puncture wounds. Tying or capping rebar can lead
to tendon inflammation (tendonitis) in the hand, wrist, or shoulder,
or carpal tunnel syndrome.

HAZARD CONTROLS:
1. Avoid cutting tie wires close to the pinch point head, instead
twist it off to eliminate sharp edges.
2. Use a rebar-tying tool which is a battery-powered equipment
that fastens rebar together with tie wires. It reduces stooped
postures by allowing the user to remain in an upright position
while stressful hand and wrist activity is minimized by the tool
automatically fastening the tie wires.
3. Wear correct PPE. Use sturdy boots and protective gloves to
avoid cuts or puncture wounds.

HAZARD DESCRIPTION:
Working under hot weather without proper clothing could result in
dehydration and dizziness. Without wearing prescribed personal
protective equipment could put workers into high risks.

HAZARD CONTROLS:
1. Adequate supply of cold drinking water must be provided to the
work area.
2. Suitable work clothing must be wear e.g. loose light-colored
clothing.
3. Work in shaded areas or erect temporary cover.
