Dear Madame,

Greetings!

As part of the partial fulfillment of the course Work Study and Measurement and
Supply Chain Management, we, the Industrial Engineering students from
Pamantasan ng Cabuyao would like to apply for a minimum of 50 hours industry
application to conduct Motion and Time Study in your company, particularly in
the Production Department. The key objectives of this field study are as follows:

1. To have a first-hand outcome-based learning of Work Study and

Measurement and Supply Chain Management in the industry through
Systems Studies and Value Stream Mapping.
2. To contribute to key improvements in the department assigned, by
applying different principle of both courses in the conduct of study.

This activity will introduce and expose us to the actual performance of workers of
their tasks and the kind of job that we are about to enter as Industrial Engineers.
Thus, we request that we conduct the study every Tuesday, Wednesday, and
Friday of the week. Tuesday and Friday will be at 1:00pm to 5:00pm and
Wednesday will be at 8:00am to 4:00pm respectively.

Your accommodation and assistance on this matter will be greatly appreciated.

treated confidentially and that we would follow the existing rules and regulations
of the company.
