This document summarizes a study analyzing accidents at a construction company called PT. X using the 5 Why method and fishbone diagrams. The study aimed to understand how effective occupational safety and health (OSH) practices were being applied at the company. By analyzing 10 accidents across two divisions, the study found that the most common causes were lack of worker concentration, failure to use personal protective equipment, and limited work spaces. The 5 Why method and fishbone diagrams helped identify underlying human, machinery, material, environmental, method, and measurement factors that contributed to each accident.

This study focuses on the analysis of the application of OSH (Occupational Safety and Healthy) in PT. X through observation of several accidents that occured during the study. This general purpose of this study is as especially students to be able to analyze how the application of effective OSH in the company. The specific purpose of this research is to know the cause of accidents that occured in the service departement of PT. X and to prepare a work safety analysis (JSA) specifically for the workshop in a effort to improve OSH for the future. The research was conducted by qualitative approach based on secondary data in the form of observation report documents, questionnaire to construction workers. This research will use the RCA method with help 5 why analysis and fishbone diagram. Result from 5 why analysis show facts cause accidents in a structured manner from both victims and other witnesses. While the results of fishbone diagram shows the cause of accident factors of each event include human factors, machinery, ingredients, environtment, methods, and measurement so as to facilitate the determination of the underlying factors of the 10 accidents analyzed in which 4 cases in the general engineering division and 6 cases in the commercial merchant division were obtained the most common cause of unsafe action is lack of concentration in work and no using PPE and the cause of unsafe conditions include limited worker space.
