“LAST MILE TOWARDS UHC: SAAN MANG SULOK, KALUSUGAN MAAABOT”

BARANGAY PINEDA, PASIG CITY

I. RATIONALE:

The Department of Health, has decided to embark on a more focused implementation of critical
health intervention. With its new battle cry “Boosting Universal Health Care via FOURmula
One Plus” otherwise known as F1 Plus aims to ensure that all Filipinos achieve better
timelines, responsiveness, and the best quality health outcomes for patients while being
protected from health inequities. To further ensure that all Filipinos are being provided with
the best health services, the Department of Health, in coordination with local government units,
is bound to provide affordable and quality health services to the marginalized population
through a series of health outreach caravans.

Henceforth, the Department of Health – National Capital Regional Office (DOH-NCRO) in

partnership with the Local Government Units (LGU), and partner DOH hospitals, will conduct
the Health Outreach for People Empowerment with the aim to reach the most vulnerable groups
who are in need of immediate health services. During its implementation, the target population
of HOPE was the National Household Targeting System for Poverty Reduction (NHTS-PR)
beneficiaries.

However, in order to reach more families and to ensure that every Filipino including the
underprivileged who are not listed under NHTS-PR has access to health services, HOPE will
be introduced to the beneficiaries of the Modified Conditional Cash Transfer (MCCT).

II. OBJECTIVES:

 To introduce the Health Outreach for People Empowerment to the MCCT beneficiaries
 To provide health services and promote health awareness to the MCCT beneficiaries
 To facilitate the involvement and participation of the different partners (DSWD,
PhilHealth, LGU) during the conduct of HOPE to the MCCT beneficiaries

III. METHODOLOGY

An approximate of 700 to 750 MCCT beneficiaries from Barangay Pineda will be provided
with the following services:

A. Registration
Public Health Nurses (PHN) with the representatives from Forward Barangay Pineda will man
the registration at the entrance of the venue. The participants will sign the registration sheet
with their name, age, sex, address and signature. Upon completion, the participants will be
given risk assessment forms.

B. Anthropometric Measurements
Body measurements including height, weight and waist and hip circumference and blood
pressure will be taken. Then, the Body Mass Index and waist-hip ratio will be computed. The
PHNs will write the measurements and the results on the risk assessment form. From here, the
senior citizens and persons with disability will be directed to the SC/PWD post, the pregnant
women to the pregnant post and the others to the regular post.
C. Risk Assessment
All 25 years old and above will proceed to this station. The PHNs with the aid of the Public
Health Associates (PHA) will accomplish the risk assessment of the participants.

D. Medical Consultation (Triage)

All participants will proceed to this station for an initial assessment with the Public Health
Nurses under the supervision of doctors from the participating hospitals. From here, they will
be directed to which ever station will apply.

E. Child care
Representatives from the Valenzuela medical team will provide pediatric services to children.

F. Surgery Referrals and Minor Surgery

The participants needing surgery such as circumcision will be profiled and screened in this
station. Physicians from East Avenue Medical Center will perform minor surgeries and will
be assessing the patients for referral of major surgeries

G. Dental Services
Dentists will provide dental services such as consultation and tooth extraction through the
mobile dental vans.

H. Exit
The participants will then proceed to the exit for the distribution of snacks and collection of
