 Please take fasting ± 10 hour prior to implementation of the medical test, but plain water is OK.
 Please bring along with you your Identity Card.
 All the information below is subject to medical checkup result.

2. Letter of Agreement:
a. The draft of agreement will be emailed by CAP to all the grantee for pre-reading purpose on tentatively
Wednesday, 20 June 2018.
b. The student is requested to provide letter from Direktorat Kemahasiswaan POLINEMA stating current
status of free-scholarship at the latest of 25 June 2018 to: beasiswapolinema@capcx.com.
c. The schedule of Agreement Signing Ceremony at POLINEMA will tentatively on 2 July 2018.

3. Scholarship fund:
a. The first allowance will be transferred thru Bank Mandiri within 4th week of July 2018.
b. Along with 1.b., please send the scanned-copy of last payment receipt of UKT and your Bank Mandiri
account number at the latest of 25 June 2018 to email: beasiswapolinema@capcx.com.
