Ethnic Group: Talaandig

Occupation: Unemployed

B. History of Present Illness


3 years ago, she experienced Polyuria, Polydipsia, Polyphagia and
verbalized weight loss as evidenced by prominent loss of
subcutaneous tissue. After few days of experiencing such condition,
she visited at the German Hospital and was diagnosed with Type 2
Diabetes mellitus. The patient was prescribed maintenance
medication and able to comply the medication given. One year after
admission, the patient worked at Valencia city as housemaid, then
they had a misunderstanding towards her boss. She experienced a
stressful feeling afterwards. Then, she felt dizziness, severe
headache, and blurring vision. On that day, she ignored her
condition. The next morning, she went home and still felt dizziness,
severe headache and blurring vision accompanied by numbness,
tingling, weakness on her right side of the body, and had a loss of

consciousness. Due to this condition, she was rushed by her
daughter to the Bukidnon Provincial Hospital – Maramag, Bukidnon
and was diagnosed cerebrovascular accident and hypertension.
Then she stayed at the hospital for about 7 days. Since then, the
patient was being immobilized due to right side body weakness, lack
of exercise and had loss of appetite.
C. Past Health History
1. Childhood Illnesses

The patient had serious illness such as mumps and chicken pox
when she’s still young. She had experienced fever and colds too.

2. Serious Illness

She was diagnosed with hypertension last 3 years ago at German


Hospital and she was then diagnosed with type 2 diabetes mellitus and
cebrovascular accident last 2 years ago at Bukidnon Provincial Hospital
- Maramag

3. Immunizations

E. Adarna hasn’t received any vaccines since birth. According to her


vaccines were unfamiliar during his time.

4. Surgeries

The patient hasn’t undergone any surgery since birth.

5. Allergies

The patient doesn’t have any known allergies.
