2. A non-refundable registration fee of Rs. 4000/team (male) and Rs. 2000/team (female) must
be submitted along with this form to confirm registration.
3. GACS Management Committee reserves the right to disqualify any team or not to register any
team. The decision of the committee would be final.
4. Attach the age proof of players ( B form for U-17 and NIC copy for U-22 players) cutting and
over writing is not acceptable.
5. Attach passport size photo with the player age proof . Gacs Futsal Fest Committee will provide
the team cards to each team. Without Team card Player Cannot be enter in the ground.
