I, SWEEZY M. CRUZ, of legal age, Filipino and a resident of

247 Magallona St., Tanjay City, Negros Oriental, after having been
sworn to in accordance with law, depose and say:

1. I am the Plaintiff in the above-entitled case.

2. I have caused the preparation and filing of the foregoing

complaint by my counsel; and have duly read and understood
all its contents and found the allegations therein to be true and
correct based on my personal knowledge and on authentic
record;

3. I have not commenced any similar action or proceeding

involving the same issues in the Supreme Court, Court of
Appeals or any other courts, tribunal or agency;

4. To the best of my knowledge, no such action or proceeding is

pending in such courts, tribunal or agency; should I thereafter
learn of such fact, I undertake to instantly notify the court within
5 days from knowledge.
